摘要
目的探讨经脐单孔腹腔镜在子宫肌瘤剔除术中的应用效果。方法选取2017年4月至2019年2月在本院接受子宫肌瘤剔除术的患者88例作为研究对象,采取随机表法分为对照组与观察组,各44例。对照组采用多孔腹腔镜进行手术治疗,观察组采用经脐单孔腹腔镜进行手术治疗,比较两组患者的术中出血量、手术时间、术后住院时间、术后24 h疼痛情况评分、切口美观满意度评分、术前术后生活质量评分以及术后并发症发生率。结果观察组手术时间长于对照组,术后住院时间短于对照组(P<0.05);术后24 h疼痛情况评分以及切口美观满意度评分均优于对照组;且术后两组患者的生活质量评分均提高,且观察组的生活质量评分高于对照组(P<0.05);观察组的术中出血量以及术后并发症发生率与对照组比较差异均无统计学意义。结论在子宫肌瘤剔除术中采用经脐单孔腹腔镜治疗较多孔腹腔镜治疗效果更优,能够有效减轻患者疼痛,减少术后并发症,从而有效缩短患者康复进程,患者认可度更高,值得临床推广应用。
Objective To investigate the effect of transumbilical single hole laparoscopy in myomectomy.Methods A total of 88 patients who underwent myomectomy in our hospital from April 2017 to February 2019 were selected as the study subjects.They were divided into control group and observation group,with 44 cases in each group.Control group used porous laparoscopic surgery,and observation group used umbilical single-arch laparoscopic surgery treatment.The bleeding volume,operation time,hospital stay,pain score,incision aesthetic satisfaction score,preoperative and postoperative quality of life score and postoperative complications rate were compared between the two groups.Results The operative time of the observation group was longer than that of the control group,and the postoperative hospitalization time was shorter than that of the control group(P<0.05).The scores of postoperative pain and aesthetic satisfaction were better than those of the control group at 24 h.In addition,the scores of quality of life in both groups were improved after the operation,and the scores of quality of life in the observation group were higher than those in the control group(P<0.05).The difference of intraoperative blood loss and postoperative complication rate between the observation group and the control group was not statistically significant.Conclusion In myomectomy,transumbilical single-port laparoscopic therapy is more effective than multi-hole laparoscopic therapy,which can effectively reduce pain and postoperative complications,thus effectively shorten the recovery process of patients,patients'recognition is higher,and it is worthy of clinical application.
