摘要
目的探讨经脐单孔腹腔镜技术在异位妊娠、附件囊肿手术中应用的可行性及安全性。方法选择2013年6月-2016年12月在我院住院的异位妊娠、附件囊肿患者32例，其中经脐单孔腹腔镜组16例为研究组，由同一主刀医师实施的同病种传统三入路腹腔镜组16例为对照组，分析比较两组手术时间、术中出血量、术中标本取出时间、术后美容满意度、视觉模拟评分法（VAS）评分等方面的差异。结果32例手术均获成功，两组术前包块大小、手术时间、术中出血量的差异无统计学意义（P〉0．05）。研究组术中标本取出时间短于对照组，术后2周切口美容评分高于对照组，术后第1天VAS评分低于对照组，差异均有统计学意义（P〈0．05）。结论经脐单孔腹腔镜能够完成异位妊娠、附件囊肿手术，术中出血少，对手术体位要求低，减少麻醉风险，标本取出容易、完整，减少残留风险，安全可行；因切口隐蔽，患者满意度较高。
Objective To investigate the feasibility and safety of transumbilical laparoendoscopic single - site sur- gery in ectopic pregnancy and uterine appendage cysts. Methods A total of 32 patients who were admitted in our hospi- tal due to ectopie pregnancy and uterine appendage cysts from June 2013 to December 2016 were selected. Then, 16 pa- tients underwent transumbilical laparoendoseopic single -site surgery and set as a research group, while another 16 pa- tients were treated along traditional three - port laparoscopic approach by the same surgeon and set as a control group. Both groups were compared for operation time, intraoperative blood loss, the time for sample taking during operation, postoperative cosmetic satisfaction, and VAS score. Results All the operations were successfully performed. There was no significant difference between the two groups in mass size before operation, operation time, intraoperative blood loss （ P 〉 0.05 ）. The research group showed shorter time for sample taking, higher scores of cosmetic satisfaction two weeks after operation, and lower VAS one day after operation, compared the control （P 〈 O. 05 ）. Conclusions Transumbilical laparoendoscopic single - site surgery can be adopted for ectopic pregnancy and ovarian cyst, with little blood loss during operation, low requirement for surgical position, and a low risk of anesthesia. It is easy and safety to completely remove the specimen with less residual risk. With concealed incisions, the patients are satisfactory with the operation.
