摘要
目的分析大理大学第一附属医院住院患者抗菌药物临床应用情况,为临床合理用药及医院抗菌药物品种遴选提供依据。方法通过医院信息系统(HIS)调取2018年1-12月大理大学第一附属医院住院抗菌药物的相关信息,包括药品名称、总使用量和销售金额等,计算用药频度(DDDs)、限定日费用(DDC)及药品销售金额排序(B)/DDDs排序(A)等数据。结果2018年医院住院患者使用抗菌药物DDDs排名前20位的抗菌药物,非限制使用级抗菌药物及限制使用级抗菌药物占比80%,特殊使用级抗菌药物占比20%,抗菌药物使用品种结构合理。B/A接近1的抗菌药物有11种,>1.5有6种,<0.5有3种;大多数药物的销售金额与DDDs同步情况良好,占有市场份额与DDDs均较高,兼有较好的社会效益和经济效益。结论医院抗菌药物品种使用结构较为合理,但点评中还存在一些不合理用药情况,还需加强监管,针对不合理用药情况进行干预,加强抗菌药物知识及政策的培训,以期提高医院的抗菌药物使用水平。
Objective To analyze the clinical application of antibacterial drugs in inpatients of the First Affiliated Hospital of Dali University,to provide a basis for rational clinical use of drugs and selection of hospital antibacterial drugs.Methods The hospital information system(HIS)was used to obtain the relevant information of antibiotics in the First Affiliated Hospital of Dali University in 2018,including drug name,total usage and sales amount,etc.Calculated DDDs,DDC and drug sales amount sorting(B)/DDDs sorting(A)and other data.Results In 2018,inpatients in our hospital used antimicrobials ranked in the top 20 in DDDs,non-restricted-grade antibacterial drugs and restricted-use antibacterial drugs accounted for 80%,special-use antibacterial drugs accounted for 20%.The variety structure of antimicrobial agents was reasonable.There were 11 kinds of antimicrobial agents close to 1,6 kinds of antibiotics greater than 1.5 and 3 kinds of antibiotics less than 0.5.The sales amount of most drugs was in good synchronization with DDDs,the market share and DDDs were both high,with good social and economic benefits.Conclusion The use structure of antibacterial drugs in hospitals is more reasonable,but there are still some unreasonable drug use in the reviews.It is necessary to strengthen supervision,intervene in the case of unreasonable drug use,and strengthen the training of antibacterial drugs knowledge and policies,in order to improve the level of antibacterial drugs in hospital.
