摘要
目的:通过对不同类型过敏性紫癜患儿的血小板参数和淋巴细胞指标变化情况进行测定,以此作为依据判断患儿患病严重程度。方法:本文40例研究对象均选自我院于2018年1月至2019年9月期间收治的过敏性紫癜患儿,根据过敏性紫癜患儿不同的临床表现将患儿分为五个组别,即单纯型(13例)、关节型(8例)、腹型(6例)、肾型(5例)和混合型(8例)。利用血液细胞分析仪对患儿PLT、MPV、PDW、PCT等进行测定,利用流式细胞技术对患儿外周血T淋巴细胞(CD3^+、CD4^+、CD8^+、CD4/CD8)水平进行测定,并且对比分析。结果:关节型、腹型和混合型过敏性紫癜患儿PLT、PCT水平较单纯型和肾型过敏性紫癜患儿水平增高,差异有统计学意义(P<0.05);关节型、腹型和混合型过敏性紫癜患儿CD3^+、CD4^+比例以及CD4/CD8比值较单纯型和肾型过敏性紫癜患儿降低,差异有统计学意义(P<0.05);五种类型过敏性紫癜患儿间MPV、PDW、CD8^+水平无差异,差异无统计学意义(P>0.05)。结论:过敏性紫癜患儿的血小板参数变化的同时,T淋巴细胞指标也发生变化且功能出现紊乱。在针对过敏性紫癜患儿进行治疗过程中,要注意调节免疫,避免感染对患儿免疫功能的影响,患儿的PLT、PCT参数明显升高或CD3^+、CD4比例和CD4/CD8比值降低时,注重患儿关节和腹部症状,反之则需注意患儿肾功能变化。
Objective:To determine the changes of platelet parameters and lymphocyte indexes in children with different types of allergic purpura,as a basis to determine the severity of the disease.Methods:The 40 research subjects in this article were selected from children with allergic purpura admitted to our hospital from January 2018 to September 2019.The children were divided into five groups according to their different clinical manifestations.That is simple type(13 cases),articular type(8 cases),abdominal type(6 cases),kidney type(5 cases)and mixed type(8 cases).Using blood cell analyzer to measure PLT,MPV,PDW,PCT,etc.of children,using flow cytometry to measure the level of peripheral blood T lymphocytes(CD 3^+,CD 4^+,CD 8^+,CD 4/CD 8)in children,and comparative analysis.Results:The levels of PLT and PCT in children with arthritic,abdominal and mixed allergic purpura were higher than those in children with simple and renal allergic purpura.The ratio of CD 3^+,CD 4^+and the ratio of CD 4/CD 8 in children with mixed anaphylactoid purpura was lower than that in children with simple and renal anaphylactoid purpura,the difference was statistically significant(P<0.05);MPV among children with five types of anaphylactic purpura,PDW and CD8+levels were not different,and the difference was not statistically significant(P>0.05).Conclusion:At the same time that the platelet parameters of children with allergic purpura change,the T lymphocyte index also changes and the function is disordered.In the treatment of children with allergic purpura,attention should be paid to regulating immunity to avoid the impact of infection on the immune function of the child.When the PLT and PCT parameters of the child are significantly increased or the CD 3^+,CD 4 ratio and CD4/CD8 ratio are reduced,Pay attention to the symptoms of the joints and abdomen of the child,and vice versa.
