高压氧联合西洛他唑治疗糖尿病足溃疡的临床疗效及对相关指标的影响 被引量:1

摘要 目的评价高压氧联合西洛他唑治疗糖尿病足溃疡患者的临床疗效以及对臂踝脉搏波传导速度(baPWV)、踝肱指数(ABI)和经皮氧分压(TcPO2)等指标的影响。方法选取2016年1月~2018年12月我科确诊的糖尿病足溃疡患者84例,采用随机数字表法分为高压氧(HBO)组、西洛他唑(CLT)组和联合治疗(HBO+CLT)组,每组28例。在常规治疗的基础上,HBO组应用HBO治疗;CLT组予以西洛他唑口服治疗;联合治疗组应用HBO和西洛他唑联合治疗。治疗3个疗程后,观察糖尿病足溃疡患者的临床疗效,治疗前后baPWV、ABI、TcPO2、MCV、SCV、动脉血流速度的变化。结果经过3个疗程的治疗,联合治疗组总有效率高于HBO组和CLT组,差异均有统计学意义(P<0.05);3组患者baPWV较治疗前明显降低,ABI、TcPO2较治疗前明显升高,差异有统计学意义(P<0.05),联合治疗组较CLT组和HBO组上述指标改善更明显,差异均有统计学意义(P<0.05);3组患者MCV、SCV和动脉血流速度均较治疗前提高,差异有统计学意义(P<0.05);联合治疗组较CLT组和HBO组提高更显著,差异均有统计学意义(P<0.05)。结论高压氧联合西洛他唑治疗能有效改善糖尿病足溃疡患者的baPWV、ABI、TcPO2水平,提高神经传导速度和动脉血流速度,促进溃疡愈合,临床效果显著。 Objective To assess the clinical effect of hyperbaric oxygen combined with cilostazol in the treatment of diabetic foot ulcer and its influences on brachial-ankle pulse wave velocity(baPWV),ankle-brachial index(ABI)and transcutaneous partial pressure of oxygen(TcPO2)and other related indicators.Methods From Jan 2016 to Dec 2018,84 patients with diabetic foot ulcer were selected.According to the random number table method,they were divided into the hyperbaric oxygen(HBO)group,the cilostazol(CLT)group and the combined treatment group with 28 cases in each group.On the basis of basic treatment,the HBO group was given the hyperbaric oxygen therapy,the CLT group was given the cilostazol treatment,and the combined treatment group was given the hyperbaric oxygen combined with cilostazol treatment.After three courses of treatment,the clinical efficacy and the changes of baPWV,ABI,Tc PO2,MCV,SCV and arterial blood flow velocity were observed.Results After three courses of treatment,total effective rate of combined treatment group was significantly higher than that of HBO group and CLT group,and the difference was statistically significant(P<0.05).The baPWV of the three groups was significantly lower than that before treatment,and the ABI and Tc PO2 were significantly higher than that before treatment(P<0.05).The improvement of the above indexes in the combined treatment group was more obvious than that in the CLT group and HBO group,and the difference was statistically significant(P<0.05).MCV,SCV and arterial blood flow velocity in the three groups were all higher than those before treatment,with statistical significance(P<0.05);the combined treatment group was significantly higher than those in CLT group and HBO group,with statistical significance(P<0.05).Conclusion HBO combined with cilostazol can effectively improve the levels of baPWV,ABI and TcPO2 in patients with diabetic foot ulcer,increase nerve conduction velocity and arterial blood flow velocity,and promote ulcer healing.The clinical effect is remarkable.
