摘要
目的探讨人文关怀护理对合并抑郁的肝癌患者的生活质量、负性情绪的影响。方法选择我院2017年6月至2019年6月收治的合并抑郁的肝癌患者120例,采用数字表抽取法随机分为对照组和观察组,两组基础治疗相同,观察组(n=60)应用人文关怀护理模式,对照组(n=60)应用常规护理模式。治疗前后两组的知信行评分、炎性因子和免疫功能指标水平、生活质量评分、焦虑抑郁评分、疼痛程度分级进行比较。结果干预前,两组患者的知信行各维度评分无差异(P>0.05),干预后,两组各项评测分值均有升高,且相观察组升高程度更为显著(P<0.05)。干预前,两组炎性指标hs-CRP、IL-8水平及免疫功能指标CD3+、CD4+水平无差异(P>0.05),干预后,两组炎性指标测验值均有降低,免疫功能指标测验值均有升高,观察组变化更明显(P<0.05)。两组干预前,生活质量各维度评分无差异(P>0.05),干预后,心理功能、社会功能、躯体功能评测分值均有升高,症状反应评测分值均有降低,且观察组变化更为显著(P>0.05)。干预前,焦虑、抑郁评分经评测无差异(P>0.05),干预后,两组评测分值均有降低,观察组降低更明显(P<0.05)。观察组疼痛程度分级情况优于对照组(P<0.05)。结论合并抑郁的肝癌患者重视人文关怀护理工作的开展,可更好的改善其知信行水平,降低其机体炎性反应程度,增强免疫功能,提高生活质量,缓解焦虑抑郁情绪,减轻疼痛。
Objective To explore the influence of humanistic care on quality of life,negative emotion in liver cancer patients with depression. Methods A total of 120 liver cancer patients with depression were randomly divided into two groups :the observation group(n=60,with routine nurse care)and the control group(n=60,with humanistic care). The scores of knowledge,trust and behavior level;the level of inflammatory factors and immune function;the quality of life,the level of anxiety,depression and pain between two group were compared. Results Before intervention,there was no difference between the two groups(P>0.05). After the intervention,the scores of all dimensions of knowledge,belief and behavior,the values of immune indexes,were increased,the scores of psychological function,social function and physical function in both two groups were increased;while the values of inflammatory indexes,the scores anxiety and depression,and the pain grade were decreased(P<0.05). And the changes in study group were more significant than the control group(P<0.05). Conclusion Liver cancer patients with depression should pay attention to humanistic care,which can improve the level of knowledge,reduce the degree of inflammatory response,enhance the immune function,improve the quality of life,relieve anxiety and depression and relieve pain.
