摘要 目的探讨咖啡因不同给药时间对极低出生体重早产儿临床结局的影响。方法选取2017年1月至2019年1月入住本院新生儿科的极低出生体重早产儿(胎龄<34周、体重<1500 g)为研究对象,按照给药时间分为研究组(96例,出生72 h内开始使用枸橼酸咖啡因)和对照组(116例,出生72 h后开始使用枸橼酸咖啡因)。比较两组的氧暴露时间、气管插管时间、无创通气时间、住院时间及并发症发生情况。结果研究组气管插管时间短于对照组(P<0.05)。研究组的BPD、hsPDA发生率低于对照组(P<0.05)。结论早期咖啡因预防治疗可明显改善极低出生体重早产儿的预后,有利于早期拔管,降低BPD及hsPDA发病率。后续可进一步行前瞻性研究来确定是否对早产儿预防应用咖啡因治疗。 Objective To investigate the effect of different administration time of caffeine citrate on clinical outcome of very low birth weight premature infants.Methods From January 2017 to January 2019,very low birth weight premature infants(gestational age<34 weeks,weight<1500 g)admitted in the neonatology department of our hospital were selected as the study objects.According to the administration time,the infants were divided into study group(96 cases,starting to use caffeine citrate within 72 hours of birth)and control group(116 cases,starting to use caffeine citrate after 72 hours of birth).Oxygen exposure time,endotracheal intubation time,non-invasive ventilation time,hospital stay and complications were compared between the two groups.Results The endotracheal intubation time of the study group was shorter than that of the control group(P<0.05).The incidences of BPD and hsPDA in the study group were lower than those in the control group(P<0.05).Conclusion Early prevention treatment with caffeine citrate can significantly improve the prognosis of very low birth weight premature infants,which is beneficial to early extubation and reduce the incidences of BPD and hsPDA.Further perspective studies are needed to determine if caffeine citrate should be recommended for premature infant.
