摘要
目的整理归纳王建华教授临床辨治甲状腺癌术后郁证的学术思想和经验。方法通过跟师随诊交流和研究甲状腺癌术后郁证的临床病例资料,从病理因素、核心病机、临证治则治法以及遣方用药等方面总结王建华教授治疗甲状腺癌外科术后郁证的学术思想和心得体会,同时予典型医案加以佐证。结果王建华教授认为甲状腺癌术后郁证的主要病因是气、火和痰,三者相互联系。病机关键是气机郁滞,久而化火,或因气郁成痰,或炼液为痰,后期火灼阴血,最终阴虚血亏,虚实夹杂。对于本病应辨证结合辨病论治,治疗上须调畅气机,以疏肝为要,治心乃变通之法,心肝兼治,疏肝宁心;同时予化痰解郁,并根据患者临床四诊资料随症加减。结论王建华教授治疗甲状腺癌外科术后郁证具有丰富的经验,特色鲜明,其研究配伍的疏肝宁心汤具有确切疗效,值得借鉴学习。
Objective To summarize Professor WANG Jianhua’ s academic thoughts and experience in clinical diagnosis and treatment of postoperative depression syndrome of thyroid cancer. Methods By communicating with doctors and studying the clinical data of postoperative depression syndrome of thyroid cancer,the author summarized PROFESSOR Wang Jianhua’ s academic thoughts and experience on treating postoperative depression syndrome of thyroid cancer from the aspects of pathological factors,core pathogenesis,clinical syndrome therapy and prescription drugs to provide the typical medical cases for support. Results According to WANG Jianhua,the main causes of postoperative depression of thyroid cancer are Qi,fire and phlegm. The key to the pathogenesis is the stagnation of Qi,which turns into fire for a long time. For this disease,the treatment should be combined with the treatment of disease differentiation. The treatment must be regulating Qi and soothing live. The method of treating heart is a flexible method. According to the clinical data of the patients,the four diagnoses were added and subtracted. Conclusion PROFESSOR WANG Jianhua has rich experience and distinct characteristics in the treatment of depression syndrome after thyroid cancer surgery.
