摘要
目的研究比较经皮椎体成形术(percutaneous vertebroplasty,PVP)、经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗多节段老年骨质疏松性脊柱压缩骨折的临床疗效。方法方便选择该院2017年1月-2019年1月接受手术治疗的100例多节段老年骨质疏松性脊柱压缩骨折患者作为研究对象,按照随机数字表法分为PVP组、PKP组(每组50例),PVP组选择经皮椎体后凸成形术,PKP组选择经皮椎体成形术,两组比较疼痛评分、脊柱功能障碍评分、伤椎高度、Cobb角、骨水泥渗漏发生率、椎体高度再次丢失量。结果手术后,组间疼痛评分、脊柱功能障碍评分、伤椎高度、Cobb角比较,差异无统计学意义(P>0.05)。术后骨水泥渗漏发生率PKP组为2%,低于PVP组14%(χ~2=4.891,P<0.05)。术后随访6个月,PKP组的椎体高度再度丢失量(1.72±0.59)mm大于PVP组(1.14±0.37)mm(t=5.889,P<0.05)。结论 PVP术、PKP术用于多节段老年骨质疏松性脊柱压缩骨折治疗中,均可有效减轻疼痛和脊柱功能障碍,恢复伤椎高度,缩小脊椎后凸角度。
Objective To compare the clinical efficacy of percutaneous vertebroplasty(PVP) and percutaneous kyphoplasty(PKP) in the treatment of multi-segment osteoporotic spinal compression fractures. Methods A total of 100 patients with multi-segment osteoporotic spinal compression fractures who underwent surgical treatment from January 2017 to January2019 in the hospital were convenient selected as the research subjects. They were divided into PVP group and PKP group(each group according to the random number table method),(each group of 50 cases), percutaneous kyphoplasty was selected in the PVP group, and percutaneous vertebroplasty was selected in the PKP group. The two groups compared the pain score,spinal dysfunction score, injured vertebral height, Cobb angle, and incidence of bone cement leakage, the vertebral height loss again. Results After surgery, there were no statistically significant differences in pain scores, spinal dysfunction scores,vertebral heights, and Cobb angles between the groups(P>0.05). The incidence of postoperative bone cement leakage was2% in the PKP group, which was lower than 14% in the PVP group(χ~2=4.891, P<0.05). After 6 months of follow-up, the vertebral height loss in the PKP group(1.72±0.59)mm was greater than that in the PVP group(1.14±0.37)mm(t=5.889, P<0.05). Conclusion PVP and PKP surgery can effectively reduce pain and spinal dysfunction, restore the height of injured vertebrae, and reduce the angle of kyphosis in the treatment of multi-segment osteoporotic spine compression fractures.
