摘要
目的:对比分析经皮椎体后凸成形术(PKP)与经皮椎体成形术(PVP)治疗骨质疏松性椎体压缩性骨折的疗效及并发症。方法:以收治的50例骨质疏松性椎体压缩性骨折患者为研究对象,随机分为PKP组和PVP组,每组各25例,观察指标包括手术时间、骨水泥注入量、伤椎Cobb角度减少量、术后伤椎高度增加量,比较患者术前和术后的VAS评分和Oswestry评分及术中术后并发症。结果:经PKP治疗患者的手术时间、骨水泥注入量、伤椎Cobb角度减少量、伤椎高度增加量较经PVP手术治疗的患者均显著增加(P<0.01)。术后VAS评分较PVP组也显著降低(P<0.05)。在并发症方面,经PVP手术治疗的患者出现术中并发症骨水泥渗漏6例(24%),神经系统损伤1例,邻近椎体骨折3例,脊髓压迫2例,而PKP组的患者发生并发症概率低于PVP组,仅有2例发生骨水泥渗漏(8%),2例邻近椎体骨折(P>0.05)。结论:PKP用于骨质疏松性压缩性骨折具有有效恢复伤椎高度、矫正后凸畸形、减少骨水泥渗漏等优势,适用于发生新鲜骨折的患者。
Objective:To compare and analyse the curative effect and complications of percutaneous vertebroplasty versus percutaneous kyphoplasty on osteoporotic vertebral compression fractures.Methods:50 patients with osteoporotic vertebral compression fractures in our hospital were selected as the study samples.They were divided into PVP group and PKP group randomly with 25 cases in each group.Operation time,the injection amount of bone cement,the reduction of Cobb’s angle and the vertebral height changes werecompared between the two groups.The visual analogue scores(VAS),the oswestry scores as well aspostoperative and intraoperative complications were also evaluated.Results:The operation time,the injection amount of bone cement,the reduction of Cobb’s angle and the increased amount of vertebral height in the PKP group increased greatly significantly(P<0.01)than those in the PVP group.VAS scores after operation in the PKP group were lower than that in the PVP group(P<0.05).Besides,intraoperative complications including 6 PMMA leakage occurred in PVP group(24%),which was higher than that in the PKP group(8%)((P>0.05).Also three patients underwent adjacent vertebral fracture,twopatients underwent spinal compression and one patient underwent nervous system injury.Obviously,the incidence rate of postoperative complications in the PVP group was higher than that in the PKP group(only two cases underwent adjacent vertebral fracture).Conclusion:PKP is better than PVP due to its good recover effects on vertebral height and kyphosis aswell as less bone cement leakageespecially for patients with fresh fracture,so it deserves extensive application.
