摘要
目的探讨经阴道三维子宫输卵管超声造影术(three-dimensional hyterosalpingo-contrast-sonography,3D-HyCoSy)对不孕症患者的诊断价值,并对造影过程中发生的不良反应情况进行研究。方法收集2017年6月~2019年3月于我院就诊的52例女性不孕症患者资料,所有患者均行3D-HyCoSy及腹腔镜通染液术(Laparoscopy and dye test,CLP)评估输卵管通畅性,以CLP为金标准,分析3D-HyCoSy检测输卵管通畅性的诊断价值,并在造影过程中同时观察并记录患者发生的不良反应情况。结果以CLP检测结果为金标准,3D-HyCoSy检测输卵管通畅性有较高诊断价值,Kappa分析结果显示这两种检查方法的一致性程度较高(Kappa值为0.78,Kappa>0.61表示一致性较好),总体符合率为85.6%,两者比较,差异无统计学意义(χ^2 =0.124,P>0.05)。输卵管通畅的患者不良反应发生率明显低于不通畅患者,差异有统计学意义(P<0.05)。结论3D-HyCoSy能够真实地展现输卵管的形态及走行,并能快速、准确地判断输卵管的通畅性,在输卵管源性不孕症患者的诊断中具有较高的临床应用价值,同时我们也应该提高对检查过程中患者发生不良反应情况的认识,尽可能减少不良反应的发生。
Objective To investigate the diagnostic value of transvaginal three-dimensional hyperosalpingo-contrast-sonography(DD-HyCoSy)in the patients with infertility,and to study the adverse reactions occurred during the angiography.Methods The data of 52 female patients with infertility who were treated in our hospital from June 2017 to March 2019 were collected.All patients were given 3D-HyCoSy and laparoscopy and dye test(CLP)to evaluate tubal patency.The CLP was taken as the gold standard to analyze the diagnostic value of 3D-HyCoSy in the detection of tubal patency.The adverse reactions occurred on the patients were simultaneously observed and recorded during angiography.Results With the CLP test results as the gold standard,3D-HyCoSy has a high diagnostic value for detecting tubal patency.Kappa analysis showed that the two methods had higher compliance(Kappa value was 0.78,Kappa>0.61 indicating better compliance).The overall compliance rate was 85.6%.There were no statistically significant differences between the two methods(χ^2 =0.124,P>0.05).The incidence rate of adverse reactions in the patients with tubal patency was significantly lower than that in patients with obstructed patency,and the difference was statistically significant(P<0.05).Conclusion 3D-HyCoSy can truly display the shape and direction of the Fallopian tube,and can quickly and accurately determine the patency of the Fallopian tube.It has a high clinical application value in the diagnosis of patients with Fallopian tube-derived infertility.At the same time,we should also raise awareness of the adverse reactions of patients during the examination,so as to minimize the occurrence of adverse reactions.
