摘要
目的探讨夹脊电针结合减重步行训练(BWSTT)对脊髓损伤大鼠髓鞘超微结构及磷酸化肌球蛋白轻链(p-MLC)的影响。方法将60只大鼠随机分为正常组、模型组、电针(EA)组、药物组和EA+运动组,每组再分连续治疗7 d和14 d的2个亚组。除正常组外,其余4组运用改良的Allen法制备脊髓损伤模型,造模成功后,正常组和模型组均不予治疗,EA组采取针刺夹脊穴进行电针治疗,药物组采用腹腔注射法舒地尔(Fasudil)进行治疗,EA+运动组采用夹脊穴电针联合部分重量支撑平板训练进行治疗。各组分别连续治疗7 d和14 d,治疗结束后,通过脊髓损伤行为学评分(BBB评分)评估大鼠的后肢运动功能恢复情况,运用Western blot法检测p-MLC和中枢髓鞘来源的神经生长抑制受体(NGR)蛋白的表达,采用髓鞘染色(LFB染色)观察髓鞘的超微结构。结果BBB评分显示,脊髓损伤后大鼠后肢运动功能显著下降,治疗7 d、14 d后,EA+运动组与模型组比较,差异均具有统计学意义(P<0.05),治疗14 d后,EA+运动组BBB评分较前升高,与EA组比较,差异具有统计学意义(P<0.05)。Western blot结果显示,脊髓损伤后p-MLC、NGR明显增多,治疗7 d、14 d后,EA+运动组p-MLC和NGR表达与模型组比较,差异具有统计学意义(P<0.05);治疗14 d后EA+运动组p-MLC和NGR表达下降,与EA组比较,差异均具有统计学意义(P<0.05)。LFB染色显示,脊髓损伤后,轴突髓鞘结构遭到破坏,脊髓白质纤维紊乱,髓鞘缺失,有髓神经纤维较少,髓鞘染色平均IOD值下降明显,治疗7 d、14 d后,EA+运动组与模型组比较,差异具有统计学意义(P<0.05),治疗14 d后,EA+运动组平均IOD值较前升高,与EA组比较,差异具有统计学意义(P<0.05)。结论夹脊电针结合BWSTT可通过抑制脊髓损伤后MLC的磷酸化及NGR的表达,保护髓鞘结构,促进运动功能恢复。
Objective To investigate the effect of Huatuojiaji point electroacupuncture plus body weight support treadmill training(BWSTT)on myelin ultrastructure and phosphorylated myosin light chain(p-MLC)in rats with spinal cord injury.Method Sixty rats were randomized to normal,model,EA,medicine and EA﹢exercise groups.Each group was again divided into two subgroups respectively given consecutive treatment of 7 and 14 days.A model of spinal cord injury was made using the modified Allen method in the other four groups except the normal group.After successful model making,the normal and model groups received no treatment;the EA group,Huatuojiaji point electroacupuncture;the medicine group,intraperitoneal injection of Fasudil;the EA﹢exercise groups,Huatuojiaji point electroacupuncture plus body weight support treadmill training.The two subgroups of each group were given consecutive treatment of 7 and 14 days respectively.At the end of treatment,rat hind limb motor function was assessed according to the Basso-Beattie-Bresnahan locomotor rating scale score(BBB score),the expressions of p-MLC and central myelin-derived nerve growth inhibitory receptor(NGR)were detected by Western blot and myelin ultrastructure was observed by myelin staining(LFB staining).Result The BBB score showed that rat hind limb motor function decreased significantly after spinal cord injury and had a statistically significant difference in the EA﹢exercise group after treatment of 7 and 14 days compared with the model group(P<0.05).After treatment of 14 days,the BBB score increased in the EA﹢exercise group compared with before and had a statistically significant difference compared with the electroacupuncture group(P<0.05).Western blot results showed that p-MLC and NGR expressions increased significantly after spinal cord injury and had statistically significant differences in the EA﹢exercise group after treatment of 7 and 14 days compared with the model group(P<0.05).After treatment of 14 days,p-MLC and NGR expressions decreased in the EA﹢exe
