摘要
目的:观察头穴长留针结合体针半刺法对痉挛型脑瘫患儿肌张力及粗大运动功能和脑部血流影响。方法:选取2015年6月~2018年6月间在本院接受治疗的痉挛型脑瘫患儿128例,随机数字表法分成2组,各64例。对照组行康复治疗,观察组在对照组基础上行头穴长留针结合体针半刺法,观察患儿治疗前后脑部血流量、粗大运动功能、肌张力及表面肌电图改变状况。结果:治疗后观察组患儿血管阻力指数(RI)、血管搏动指数(PI)较治疗前、对照组降低,脑动脉收缩期峰流速度(Vs)、平均血流速度(Vm)较治疗前、对照组升高,差异均有统计学意义(P<0.05);治疗后观察组患儿A、B、C、D、E区及总得分较治疗前、对照组升高,差异均有统计学意义(P<0.05);治疗后观察组患儿平均功率频率(MPF)、均方根值(RMS)、肌电积分值(iEMG)较治疗前、对照组升高,肌张力较治疗前、对照组降低,差异均有统计学意义(P<0.05)。结论:头穴长留针结合体针半刺法可有效改善痉挛型脑瘫患儿粗大运功功能和肌张力,临床效果显著。
Objective:To analyze the effect of scalp long needle and needle acupuncture on muscle tension,gross motor function and brain blood flow in children with spastic cerebral palsy.Methods:128children with spastic cerebral palsy who were treated in our hospital from June 2015to June 2018were randomly divided into two groups.The controlled group(64patients)un-derwent rehabilitation and the observation group(64patients).In the controlled group,the apical acupuncture points and the needle acupuncture method were used to observe the blood flow,gross motor function,muscle tone and surface EMG changes be-fore and after treatment.Results:After treatment,the vascular resistance index(RI)and vascular pulsation index(PI)of the observation group were lower than those before treatment and the controlled group,and the systolic peak flow velocity(Vs)and mean blood flow velocity(Vm)were higher than before treatment and the controlled group,the difference was statistically signifi-cant(P<0.05);after treatment,the observation group A,B,C,D,E area and total score compared with before treatment and the controlled group,the difference was statistically significant(P<0.05).The average power frequency(MPF),root mean square(RMS),and myoelectric integral value(iEMG)of the observation group were higher than those before treatment and the controlled group.The muscle tension was higher than that before treatment and the controlled group,and the difference was statistically significant(P<0.05).Conclusion:The long-needle acupuncture combined with the needle acupuncture method can effectively improve the gross function and muscle tension of children with spastic cerebral palsy,and the clinical effect is re-markable.
