摘要
目的:观察健脾促愈汤治疗慢性萎缩性胃炎的临床效果及对胃动力和炎症因子的影响。方法:将100例脾胃虚弱型慢性萎缩性胃炎患者分为实验组和对照组,每组50例。所有患者采用予抑酸剂、保护胃黏膜等常规疗法治疗。对照组患者口服兰索拉唑片,实验组在对照组治疗的基础上,口服健脾促愈汤治疗。两组分别治疗3个月。比较两组治疗前后临床疗效、中医症候积分、胃黏膜萎缩、肠上皮化生和异性增生评分,胃动力情况和血清炎症因子含量。结果:研究结果显示,实验组治疗总有效率为92.00%高于对照组治疗总有效率的76.00%(P<0.05);实验组治疗后中医症候积分低于对照组(P<0.05);实验组治疗后胃黏膜萎缩、肠上皮化生和异性增生评分低于对照组(P<0.05);实验组治疗后胃泌素高于对照组(P<0.05);实验组治疗后血清炎症因子水平低于对照组(P<0.05)。结论:健脾促愈汤治疗慢性萎缩性胃炎脾胃虚弱证疗效显著,可有效的改善临床症状和胃黏膜病理,增加患者胃泌素分泌,促进胃排空,减轻胃黏膜炎症损伤,提高治疗疗效,值得临床进一步研究探讨。
Objective:To observe the clinical effect of Jianpi Cuyu Decoction on chronic atrophic gastritis and its influence on gastric motility and inflammatory factors.Methods:100cases of chronic atrophic gastritis with spleen and stomach weakness were divided into experimental group and controlled group,50cases in each group.All patients were treated with acid inhibitor and gastric mucosa protection.The patients in the controlled group were treated with Lansoprazole Tablets,while the patients in the experimental group were treated with Jianpi Cuyu Decoction on the basis of the treatment in the controlled group.The two groups were treated for 3months.The clinical efficacy,TCM symptom score,gastric mucosal atrophy,intestinal metaplasia and dysplasia score,gastric motility and serum inflammatory factors were compared between the two groups before and after treatment.Results:The results showed that the total effective rate of the experimental group was 92.00%,higher than 76.00%of the con-trolled group(P<0.05).The scores of TCM symptoms in the experimental group were lower than those in the controlled group(P<0.05).The scores of atrophy of gastric mucosa,intestinal metaplasia and heterogenous hyperplasia in the experimental group were lower than those in the controlled group(P<0.05).After treatment,gastrin level in the experimental group was high-er than that in the controlled group(P<0.05),and serum inflammatory factor level in the experimental group was lower than that in the controlled group(P<0.05).Conclusion:Jianpi Cuyu Decoction is effective in treating chronic atrophic gastritis with weakness of spleen and stomach.It can effectively improve clinical symptoms and pathology of gastric mucosa,increase gastrin secretion,promote gastric emptying,alleviate inflammation injury of gastric mucosa and improve therapeutic effect.It is worth further study and discussion in clinic.
