摘要
背景:新冠肺炎疫情,是新中国成立以来最重大的突发公共卫生事件,具有传染速度快,感染范围广,防控难度大的特点。针对此次疫情,全球的医师、药师、科学家正竭力寻找有效药物和研发疫苗,相关学科的专家依据体内外药学研究数据、药物临床试验数据和临床治疗反馈数据编写诊疗方案。然而,人类对于新冠肺炎的认知具有一定的过程,临床用药也因此存在一定风险。本文论述新冠肺炎疫情背景下的安全用药问题解决策略,为患者安全用药提供思路和借鉴。方法:通过检索关于历史疫情的用药经验和药害事件文献、新冠肺炎药物治疗安全性文献以及国家卫建委出台新冠肺炎诊疗方案和国家对于此次疫情防治的有关政策,提出突发公共卫生事件背景下临床用药存在的安全性隐患,探索新型药学服务模式,论述并总结保障安全用药的策略。结果:历史上的疫情在暴发初期往往缺乏有效和安全的治疗药物,临床用药探索可能给患者带来药物不良反应甚至药害事件。加深对疫情的认知、完善临床诊疗方案、推进药物利用研究、加强智慧药学服务、药害事件监测和新药临床试验的监管、探索新型药学服务模式、实现个体化用药,是新冠肺炎背景下保障安全用药的有效策略。结论:新冠肺炎疫情暴发以来,临床用药存在安全性担忧。新冠肺炎期间用药安全性,值得引起高度关注。应加强疫情期间药品供应管理,完善诊疗方案,发现和评价安全有效药物,加强临床试验监管和药物不良反应监测,优化药学服务模式,推广智慧药学服务,促进个体化用药,从而避免药害事件的发生,保障患者安全用药。
Backgroud:The CoronavirusDisease-2019,COVID-19 is the most significant public health emergency since the founding of People’s Republic of China,characteristics of which include rapid infection,wide range of infection,and difficult prevention and control.In response to this epidemic,physicians,pharmacists,and scientists around the world are struggling to find effective drugs and develop vaccines.Experts in related disciplines prepare diagnosis and treatment plans based on in vivo and in vitro pharmaceutical research data,drug clinical trial data,and clinical treatment feedback data.However,humans have a certain process of cognition of COVID-19,and there are certain risks in clinical use of drugs.This article discusses problems and strategies for safe use of drugs in the context of the COVID-19,and provides ideas and lessons for patients to use drugs safely.Methods:Through searching literature on historical epidemic drug use experience and phytotoxicity events,literature on the safety use of COVID-19 medications,and the guidelines of COVID-19 diagnosis and treatment and policies by National Health Commission of the People’s Republic of China for the prevention and treatment of this epidemic.Point out the hidden safety hazards of clinical medication in the context of public health emergencies,exploring new pharmacy service models,and discussing and summarizing strategies to ensure safe us of drugs.Results:Historical outbreaks of epidemics often lack effective and safe therapeutic drugs in the early stages,and clinical drug exploration may bring adverse drug reactions and even phytotoxic events to patients.Deepening understanding of the epidemic situation,improving guidelines of clinical diagnosis and treatment,advancing research on drug utilization,strengthening smart pharmacy services,monitoring of drug hazard events and supervision of new drug clinical trials,exploring new pharmacy service models,and realizing personalized medicine,are effective strategies to ensure safe use of drugs in the context of COVID-19
