循证护理在规避围术期患者低体温风险中的临床应用 被引量:4

摘要 目的探讨循证护理在规避围术期患者低体温风险方面发挥的作用,并总结经验以指导后续临床护理工作。方法取2016年4月至2017年8月期间在我院接受手术治疗的80例患者,随机分为A组35例与B组45例,A组给予外科手术常规护理,B组在常规护理基础上联合循证护理干预,比较两组术中不同时间点体温变化、不良反应发生及主要手术指标。结果B组T0、T1、T2与T3时的体温差异不显著(P>0.05);A组患者经麻醉处理后体温有降低趋势,与麻醉前均有统计学意义(P<0.05),麻醉后B组各时间点温度均高于A组(P<0.05);麻醉前两组患者血糖与血乳酸水平差异不显著(P>0.05),但T2与T3时,A组血糖与血乳酸水平较高于B组(P<0.05);围术期B组低体温、寒战发生率均低于A组(P<0.05)。结论循证护理在外科围手术期中的应用,有助于强化患者体温的相对平稳性,减少血糖与血乳酸的增加幅度,降低不良反应的发生率,促进患者术中生理功能与预后的有效改善。 Objective To explore the role of evidence-based nursing in avoiding hypothermia risk in perioperative patients and to sum up the experience to guide follow-up clinical nursing.Methods 80 patients who received surgical treatment in our hospital during the period from 2016.4 to 2010.8 were randomly divided into 35 cases in Group A and 45 cases in Group B.Group A gave routine surgical care,and Group B combined evidence-based nursing intervention on the basis of routine care.The changes of body temperature,adverse reactions and main surgical parameters were compared between the two groups.Results The temperature difference between T0,T1,T2 and T3 in group B was not significant(P>0.05);Patients in group A had a decreasing trend of body temperature after anaesthesia,which was statistically significant before anaesthesia(P<0.05).After anaesthesia,the temperature at all time points in group B was higher than that of group A(P<0.05).There was no significant difference between blood sugar and blood lactic acid levels in the two groups before anesthesia(P>0.05),but T2 and T3,blood glucose and blood lactic acid levels in Group A were higher than those in Group B(P<0.05);The incidence of hypothermia and chills in group B during perioperative period was lower than that of group A(P<0.05).Conclusion The application of evidence-based nursing in surgical perioperative period can enhance the relative stability of patient’s body temperature,reduce the increase of blood sugar and blood lactic acid,reduce the incidence of adverse reactions,and promote the effective improvement of physiological function and prognosis in patients.
