摘要
目的:探索骨伤复元汤联合负压封闭引流(VSD)在胫腓骨开放性骨折骨愈合中的应用及对患者疼痛症状的影响。方法:将80例胫腓骨开放性骨折患者随机分成观察组和对照组,每组40例。对照组采取外固定架联合VSD治疗,观察组在对照组治疗基础上联合骨伤复元汤进行治疗。对比2组切口愈合情况、疼痛、肿胀与瘀斑症状评分、疼痛消失时间、肿胀消失时间、切口愈合时间、骨痂出现时间。结果:观察组愈合效果优于对照组,差异有统计学意义(P<0.05)。2组术前的疼痛、肿胀与瘀斑评分比较,差异无统计学意义(P> 0.05)。术后2周,2组各项症状评分均较术前降低(P<0.05)。观察组术后2周的疼痛、肿胀与瘀斑评分低于对照组(P<0.05)。观察组疼痛消失时间、肿胀消失时间、切口愈合时间、骨痂出现时间均短于对照组,差异有统计学意义(P<0.05)。结论:骨伤复元汤联合VSD治疗胫腓骨开放性骨折可有效改善患者切口愈合情况,减轻疼痛、肿胀与瘀斑等症状,促进康复。
Objective:To observe the application of Gushang Fuyuan tang combined with vacuum sealing drainage(VSD) in bone healing of open tibia-fibula fractures and its effect on the pain of patients. Methods: Eighty patients with open fractures of tibia and fibula were randomly divided into the observation group and the control group,40 cases in each group.The control group was treated with external fixation combined with VSD,and the observation group was additionally treated with Gushang Fuyuan tang based on the treatment of the control group. The symptom scores of incision healing, pain,swelling and ecchymosis, and the pain disappearance time, swelling disappearance time, incision healing time, and epiphyseal appearance time were compared between the two groups. Results: The healing in the observation group was better than that in the control group,the difference being significant(P<0.05). There was no significant difference in scores of pain, swelling and ecchymosis between the two groups before surgery(P > 0.05). Two weeks after surgery, all symptom scores in the two groups were decreased when compared with those before surgery(P<0.05). Two weeks after surgery,the scores of pain,swelling and ecchymosis in the observation group were lower than those in the control group(P<0.05). The pain disappearance time, swelling disappearance time, incision healing time, and epiphyseal appearance time in the observation group were shorter than those in the control group,the difference being significant(P<0.05). Conclusion:The therapy of Gushang Fuyuan tang combined with VSD for patients with open fractures of tibia and fibula, can effectively improve the wound healing,reduce pain,swelling and ecchymosis and promote rehabilitation.
