摘要
目的观察中药活血蠲痛方与穴位注射治疗对带状疱疹后遗神经痛(PHN)患者的临床疗效,探讨其对血清白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)、前列腺素(PGE2)和β-内啡肽(β-EP)水平的影响.方法选择2018年10月至2019年10月甘肃省酒泉市人民医院收治的70例PHN患者,按治疗方法分为单纯穴位注射组〔30例,给予盐酸多柔比星及臭氧(O3)穴位注射治疗,每周1次〕和中药活血蠲痛方联合治疗组〔40例,在单纯穴位注射组的基础上加用中药活血蠲痛方(组成:板蓝根15g、蒲公英10g、地丁10g、红花10g、乳香10g、三七粉10g、黄连10g、半枝莲10g、马齿苋10g、郁金10g、乌梢蛇10g、白花蛇舌草10g)治疗,每日2次〕,两组均治疗2周为1个疗程.观察治疗前后两组的血清IL-6、TNF-α及PGE2、β-EP水平变化,统计两组的疼痛评分、烧灼评分和瘙痒评分,计算两组的总有效率以评价临床疗效.结果治疗后两组的IL-6、TNF-α、PGE2均较治疗前降低,β-EP较治疗前升高,且中药活血蠲痛方联合治疗组的改变程度较单纯穴位注射组更显著〔IL-6(μmol/L):44.42±0.33比95.63±6.45;TNF-α(ng/L):16.81±3.67比26.23±6.16;PGE2(U/L):50.32±1.71比93.23±1.38;β-EP(ng/L):25.60±6.23比19.20±3.32,均P<0.05〕.治疗后两组的疼痛评分、烧灼评分和瘙痒评分均较治疗前明显降低,且中药活血蠲痛方联合治疗组治疗后评分均明显优于同期单纯穴位注射组〔疼痛评分(分):1.28±4.52比4.54±0.02,烧灼评分(分):0.75±3.61比1.68±0.37,瘙痒评分(分):0.85±0.76比1.49±3.83,均P<0.05〕.中药活血蠲痛方联合治疗组总有效率明显高于单纯穴位注射组(95.0%(38/40)比76.7%(23/30),P<0.05).结论中药活血蠲痛方联合穴位注射治疗的活血舒筋及抑制炎性因子效果更佳,疼痛、灼烧和瘙痒改善效果更明显.
Objective To observe the clinical efficacy of Huoxue Juantong prescription(HJP)and acupoint injection in the treatment of postherpetic neuralgia(PHN)and its effects on serum levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),prostaglandin(PGE2)andβ-endorphin(β-EP).Methods Seventy PHN patients in Jiuquan City People's Hospital from October 2018 to October 2019 were selected and according to different therapies divided into two groups,such as acupoint injection group[30 cases,given adriamycin and ozone(O3)acupoint injection,once a week]and HJP treatment group[40 cases,added HJP(ingredients:Radix Isatidis 15 g,Herba Taraxaci 10 g,Corydalis bungeana 10 g,Flos Carthami 10 g,Olibanum 10 g,Notoginseng Powder 10 g,Rhizoma Coptidis 10 g,Herba Scutellariae Barbatae 10 g,Herba Portulacae 10 g,Radix Curcumae 10 g,Zaocys 10 g,Hedyotis diffusa 10 g)based on the acupoint injection group,twice daily],two weeks as a course of treatment in both groups.The serum levels of IL-6,TNF-α,PGE2,andβ-EP were observed before and after treatment.The pain score,burning score and pruritus score of the two groups were counted.The total effective rate of the two groups was calculated to evaluate the clinical effect.Results After treatment,the IL-6,TNF-α,and PGE2 in both groups decreased,β-EP increased and the HJP treatment group changed more significantly than the acupoint injection group[IL-6(μmol/L):44.42±0.33 vs.95.13±6.45,TNF-α(ng/L):16.81±3.67 vs.26.23±6.16,PGE2(U/L):50.32±1.71 vs.93.23±1.38,β-EP(ng/L):25.60±6.23 vs.19.20±3.32,all P<0.05].After treatment,the pain score,burning score and pruritus score of the two groups were significantly lower than those before treatment,and the scores of HJP treatment group were significantly better than that of acupoint injection group[pain score:1.28±4.52 vs.4.54±0.02,burning score:0.75±3.61 vs.1.68±0.37,pruritus score:0.85±0.76 vs.1.49±3.83,all P<0.05].The total effective rate of HJP treatment group was significantly higher than that of acupoint injection group[95.0%(3
