摘要
目的探究家庭护理干预在多发性骨髓瘤(MM)患者中的应用效果。方法选取MM瘤患者50例,采用随机数字表法分为对照组和研究组,每组25例。对照组给予常规出院护理,研究组采取以家庭功能为焦点的护理干预。比较2组治疗效果、骨痛缓解情况、并发症发生率及干预前后疗效相关指标[M蛋白、β2-微球蛋白(β2-MG)、血红蛋白(Hb)、骨髓浆细胞比例]、免疫功能(CD3+、CD4+、CD8+、CD4+/CD8+)、自护能力、生活质量、家庭功能变化。结果研究组总有效率、骨痛缓解率显著高于对照组,复发率、并发症发生率显著低于对照组,差异均有统计学意义(P<0.05)。干预后3个月,研究组Hb、CD3+、CD4+、CD4+/CD8+、ESCA评分、QLQ-C30评分、FAD评分高于对照组,M蛋白、β2-MG、骨髓浆细胞比例、CD8+低于对照组,差异均有统计学意义(P<0.05)。结论家庭护理干预应用于MM患者能够显著提高治疗效果,缓解骨痛,降低并发症发生率,并改善患者自护能力、生活质量及家庭功能。
Objective To investigate the application effects of family nursing intervention on multiple myeloma(MM).Methods A total of 50 patients with MM were enrolled in the study,who were randomly divided into research group(n=25)and control group(n=25).The patients in control group were treated by routine nursing intervention,however,the patients in research group were treated by family nursing intervention.The nursing effects,pain relief degree,incidence rate of complications and related indexes(M protein,β2-MG,Hb and proportion of marrow plasma cells),immune function indexes(CD3+,CD4+,CD8+and CD4+/CD8+),the self-care ability,the life quality and the family function before and after nursing were observed and compared between the two groups.Results The total effective rate and the pain relief rate in research group were significantly higher than those in control group,however,the relapse rate and incidence rate of complications were significantly lower than those in control group(P<0.05).After 3-month nursing intervention,the levels of Hb,CD3+,CD4+,CD4+/CD8+,ESCA scores,QLQ-C30 scores and FAD scores in research group were significantly higher than those in control group,but,the levels of M protein,β2-MG,proportion of marrow plasma cells and CD8+were significantly lower than those in control group(P<0.05).Conclusion The family nursing intervention can significantly alleviate the pain degree of patients with MM,decrease the incidence rate of complication,and improve the patients’self-care ability,life quality and family function.
