摘要 目的:研究海马萎缩的磁共振成像(MRI)影像分析的临床应用价值。方法:选取医院收治的50例阿尔茨海默病(AD)患者,将其纳入观察组;另选取同期于医院进行体检的50名健康体检者,将其纳入对照组。两组均接受MRI扫描,通过线性和体积测量完成海马高度、体积、颞角宽度和岛叶厚度的评估。采用成人韦氏智力量表(WAIS)和简易智能状态检查(MMSE)量表评分,对两组WAIS量表中的语言智商(VIQ)、操作智商(PIQ)和总智商(FIQ)以及认知功能进行评估,并对海马区MRI表现和认知功能指标进行相关性分析。结果:观察组海马高度、海马体积水平低于对照组,而颞角宽度水平高于对照组,其差异有统计学意义(t海马高度=4.377,t=4.419,t海马体积=13.485,t=12.163,t颞角宽度水平=5.944,t=6.066;P均<0.05);两组岛叶厚度比较差异无统计学意义。观察组WAIS量表中的VIQ、PIQ和FIQ评分以及MMSE评分均低于对照组,其差异有统计学意义(t=9.147,t=8.978,t=8.123,t=15.625;P<0.05)。海马高度、海马体积与VIQ、PIQ、FIQ及MMSE评分均呈正相关,而颞角宽度与VIQ、PIQ、FIQ及MMSE评分均呈负相关。结论:MRI影像分析可有效评估海马萎缩情况,从而为AD的早期诊断提供参考依据,且与认知功能评估存在良好的一致性,并具有一定临床应用价值。 Objective: To study the clinical application value of magnetic resonance imaging(MRI) image analysis on hippocampal atrophy. Methods: 50 patients with Alzheimer’s disease(AD) admitted to hospital were selected and were divided into observation group, and 50 healthy people who underwent physical examination in hospital during the same period were selected and were divided into control group. Both two groups underwent MRI scans to complete assessments of height, volume, width of temporal horn and thickness of insular lobe of hippocampus by linear and volume measurements. The Wechsler adult intelligence scale(WAIS) and mini-mental state examination(MMSE) were adopted to assess verbal intelligence quotient(VIQ), performances intelligence quotient(PIQ) and full intelligence quotient(FIQ) and cognitive function of WAIS in two groups, and to implement correlation analysis for the relationship between MRI appearance of hippocampus and indicators of cognitive function. Results: The height and volume of hippocampus in observation group were significantly lower than those in control group, while the width of temporal horn of observation group was significantly higher than that of control group(theight of hippocampus=4.377, t=4.419, tvolume of hippocampus=13.485, t=12.163, twidth level of temporal horn=5.944, t=6.066, P<0.05). And the difference of the thickness of insular lobe between two groups was no significant. The VIQ, PIQ AND FIQ scores of WAIS of observation group were significantly lower than those of control group(t=9.147, t=8.978, t=8.123, t=15.625, P<0.05). Pearson correlation analysis showed that the height and volume of hippocampus were positively correlative with the scores of VIQ, PIQ, FIQ and MMSE, respectively, while the width of temporal horn was negatively correlative with the scores of VIQ, PIQ, FIQ and MMSE. Conclusion: MRI image analysis can effectively assess the situation of hippocampal atrophy, thus can provide reference for the early diagnosis of AD, and it has a good consistency with the assessm
