两种不同胚胎移植管在IVF-ET中的临床应用分析 被引量:5

摘要 目的:分析Frydman移植管和PCC移植管在体外受精-胚胎移植(IVF-ET)中的临床效果。方法:回顾性分析2018年1~12月我院IVF-ET治疗中由同一医生完成的949例新鲜胚胎移植周期,根据移植管类型分为两组:Frydman组481例周期(采用进口移植管Frydman),PCC组468例周期(采用国产移植管PCC),分析两组妊娠结局、血染程度、血染位置、装载胚胎耗时、外导管置管困难率和困难移植率。结果:①两组胚胎着床率、临床妊娠率、异位妊娠率、继续妊娠率差异均无统计学意义(P>0.05)。②PCC组无血染率(84.83%)高于Frydman组(63.83%),轻度、中重度血染率和外导管血染率低于Frydman组,差异有统计学意义(P<0.05);两组内导管血染率和内外导管血染率比较,差异无统计学意义(P>0.05)。③两组胚胎装载耗时、困难移植率比较,差异无统计学意义(P>0.05);PCC组外导管置管困难率(36.75%)低于Frydman组(78.38%),差异有统计学意义(P<0.05)。结论:IVF-ET中选择不同胚胎移植管妊娠结局无差异,但国产胚胎移植管PCC有降低血染程度和外导管置管困难率的优点,具有保护子宫颈组织的作用,临床适用性较好。 Objective:To analysis the clinical effect in two embryo transfer tubes of IVF-ET.Methods:A retrospective analysis was made on 949 fresh embryo transfer cycles during IVF-ET treatment that completed by the same doctor in our hospital from January 2018 to December 2018.They were divided into two groups by the type of transplantation tube:481 cycles in Frydman group(using Frydman tube)and 468 cycles in PCC group(using PCC transplantation tube embryos),respectively.The pregnancy outcome,the degree of blood staining,the location of blood staining,the time of loading embryo,the difficulty rate of external catheter placement and the difficulty rate were analyzed between two groups.Results:① There were no significant differences in the rates of embryo implantation,clinical pregnancy,ectopic pregnancy,and continued pregnancy between the two groups(P>0.05).② The rate of no blood staining in the PCC group(84.83%)was higher than that of the Frydman group(63.83%),and the mild,moderate to severe blood staining rate and outer catheter blood staining rate were lower than the Frydman group,and the differences were statistically significant(P<0.05);There was no significant difference in the blood staining rate between the internal and external catheters in the group(P>0.05).③ There was no significant difference in the time-consuming and difficult transplantation rateof embryo loading between the two groups(P>0.05).The PCC difficulty rate of external catheterization(36.75%)was lower than the Frydman group(78.38%),and the difference was statistically significant(P<0.05).Conclusions:The application of two different embryo transfer tubes in IVF-ET have no difference on clinical pregnancy outcomes,but PCC of domestic embryo transfer tube has the advantages of reducing the degree of blood staining and difficulty in placing external catheter,protecting the cervix tissue and having good clinical applicability.
