摘要
目的比较3D打印技术辅助与传统手术治疗复杂胫骨平台骨折的疗效。方法选取2018年1月至2019年6月在甘肃省张掖市甘州区人民医院骨科就诊的46例复杂胫骨平台骨折患者作为研究对象。采用随机数字表法将其分为对照组和观察组,每组23例。对照组采用传统切开复位内固定术治疗,观察组在3D打印技术辅助下行切开复位内固定术治疗。比较两组骨折复位情况、膝关节功能及下肢步行能力,统计两组手术指标及术后并发症发生情况。结果观察组膝关节Rasmussen评分、下肢步行能力、运动功能均优于对照组,差异均有统计学意义(均P<0.05);观察组术中出血量少于对照组,手术时间、愈合时间及完全负重时间均短于对照组,并发症总发生率低于对照组,差异均有统计学意义(均P<0.05)。结论3D打印技术辅助治疗复杂胫骨平台骨折能直观反映骨折的分型和严重程度,而且通过术前在实物模型上进行虚拟手术能够获得准确的复位,与传统手术相比具有手术时间短、术后并发症少、患者恢复快等优势。
Objective To compare the 3D printing-assisted technology and traditional surgery in the treatment of complex tibial plateau fractures.Methods Forty-six patients with complex tibial plateau fractures who were treated in the Department of Orthopedics,Ganzhou District People's Hospital from January 2018 to June 2019 were selected as the research subjects.They were randomly divided into the control group and the observation group,with 23 cases in each group.The control group were treated with traditional open reduction and internal fixation.The observation group were treated with 3D printing-assisted open reduction and internal fixation.The fracture reduction,knee function,lower limb walking ability of 2 groups were compared,and the surgical indicators and postoperative complications of 2 groups were accounted.Results The knee joint Rasmussen score,lower limb walking ability and motor function of the observation group were better than that of the control group,and the differences were statistically significant(all P<0.05).The observation group had less intraoperative blood loss,shorter operation time,healing time and complete weight-bearing time,and lower total incidence of complications than the control group,and the differences were statistically significant(all P<0.05).Conclusions 3D printing assisted technology can intuitively reflect the type and severity of fractures and help surgeon perform virtual surgery on the physical model before surgery in treatment of complex tibial plateau fractures.It achieves shorter operation time,fewer postoperative complications and quicker recovery of patients compared with traditional surgery.
