3种植骨材料在上颌后牙区种植中稳定性的观察 被引量:11

摘要 目的比较3种植骨材料对种植体稳定性的影响,为临床应用提供参考。方法选择上颌后牙缺失且上颌窦底高度不足(上颌窦底至牙槽嵴顶高度约2~4 mm,平均3.1 mm)行上颌窦外提升同期种植的患者30例,将其随机分为A、B、C组,每组10例。术中植入同种型号种植体的同时均行上颌窦外提升植入并填入不同植骨材料,其中A组植入Bio-Oss骨粉,B组植入Bio-collagen骨胶原,C组植入Bio-Oss骨粉联合浓缩生长因子(concentrated growth factors,CGF)。分别于术后即刻、术后6个月(二期种植修复时)以及术后12个月(功能负重后)使用动度测量仪测量植体稳定系数(ISQ值),并对测量数据进行统计分析。对所测量的数据利用SPSS13.0软件进行统计学分析,比较不同骨移植材料在上颌后牙区骨量不足的病例中对种植体稳定性的影响。结果(1)A组在种植术后即刻、术后6个月及12个月的ISQ值分别为58.66±5.05、73.65±4.57和77.46±3.50;B组分别为57.53±4.57、72.79±5.00和74.27±5.63;C组为别为58.47±5.67、87.69±4.57和88.78±3.48;A组与B组种植术后即刻与术后12个月的ISQ值差异无统计学意义(P>0.05);(2)C组术后6个月及12个月ISQ值明显高于其余两组,差异有统计学意义(P<0.05)。结论3组植骨材料在上颌窦外提升中均可有效增加剩余骨量,且成骨效果好。Bio-Oss骨粉联合CGF在增加骨量的同时能缩短恢复期,为种植体的早期负载提供临床依据。 Objective To compare the effects of three kinds of bone implantation materials(Bio-Oss,Bio-collagen and Bio-Oss joint CGF)on the stability of implants,and to provide reference for clinical application.MethodsThirty patients,male and female,aged 30 years old,who underwent maxillary sinus elevation at the same time,were divided into three groups with 10 patients in every group.Group A was treated with Bio-Oss bone powder implantation,Group B was treated with Bio-collagen bone collagen implantation,and Group C was treated with Bio-Oss bone powder combined with concentrated growth factor(CGF).The stability of the implantation was measured and recorded by mobility meter during the operation at the first stage,6 months of the 2nd implantation stage and 12 months of implantation restoration.The stability of the planting body should be measured for several times,and the minimum value was adopted.ResultsThe ISQ values at 0,6 and 12 months after implantation of Group A were 58.66±5.05,73.65±4.57 and 77.46±3.5 respectively.The ISQ values at 0,6 and 12 months of Group B were 57.53±4.57,72.79±5.00 and 74.27±5.63 respectively.The ISQ values at 0,6 and 12 months of Group C were 58.47±5.67,87.69±4.57 and 88.78±3.08 respectively.There was no significant difference in ISQ value between Group A and B at 0 month after operation and 12 months after operation(P>0.05).The ISQ values of Group C were significantly higher than those of the other two groups at 6 and 12 months after operation(P<0.05).ConclusionAll the three groups of bone grafting materials could effectively increase the residual bone mass in maxillary sinus elevation.The Bio-Oss bone powder combined with concentrated growth factor could shorten the recovery period while increasing bone mass,which provided clinical basis for the early loading of the implantation.
