摘要
目的探讨我院中药不合理处方类型及原因并进行统计和典型案例分析,旨在促进临床合理用药。方法回顾性抽查我院2017年度中药房门诊的1080张处方,对处方进行点评、案例分析并归纳不合理处方类型,制定相关干预措施。结果不合理处方159张(占14.7%),排序居前三的科室分别为门诊、内二科、内一科。我院不合格处方大致分为3类,用药方法不适宜、超常处方及不规范处方。9个典型案例涉及给药途径不适宜、配伍禁忌、遴选药品不适宜等问题。根据不合理处方制定干预办法分为规范医师执业行为、规范药师执业行为两项。结论我院应加大处方管理力度,加强医师合理用药意识,辨证论治提高处方质量。
Objective In order to promote the rational use of medicine in clinic,to investigate the type and cause of unreasonable prescription of traditional Chinese medicine in our hospital and count the typical cases.Methods The 1080 prescriptions are reviewed and analyzed which is from the clinic of Chinese pharmacy in our hospital in 2017.The unreasonable prescription types are summarized.The relevant intervention measures are formulated.Results The 159 prescriptions were unreasonable(14.7%),and the departments which ranked the top three were outpatient,Internal MedicineⅡ,and Internal MedicineⅠ.There are three kinds of unqualifi ed prescriptions in our hospital,which are inappropriate,unusual and non-standard.There are nine typical cases which consist of inappropriate routes of administration,contraindications of compatibility,inappropriate selection of drugs and the like.According to the unreasonable prescriptions,the intervention measures are divided into two parts:the behavior of doctor should be regulated and the behavior of pharmacist should be improved.Conclusion We should to improve the prescription management and the reasonable use of rational drugs,dialectical treatment to enhance the quality of prescriptions.
