摘要
目的分析我院门诊中药饮片处方用药情况,并针对性的提出用药对策。方法从我院处方分析系统中随机抽取门诊2018年中药饮片处方1500张进行统计,分析用药情况及不合理处方类型。结果我院2018年各季度门诊不合理中药饮片处方分别为62张、67张、61张、57张,不合理率分别为4.13%、4.47%、4.07%、3.80%。1500张中药饮片处方中共有不合理处方247张(16.47%),包括重复用药(19.84%)、超剂量用药(14.17%)、用法不规范(25.10%)、配伍禁忌(16.60%)、用药不符(13.36%)、书写不规范(10.93%)。结论我院门诊中药饮片处方的不合理使用现象发生率较高,针对用药情况提出多科室进一步加强合作,医师规范处方书写,中药师在调剂前加强审核,制定合理的奖惩制度,将中药饮片处方合理使用纳入绩效考核,促进合理用药。
Objective To analyze the prescription of Chinese herbal medicines in outpatient department of our hospital,and toput forward specific countermeasures for medication.Methods From the prescription analysis system of our hospital,1500 prescriptions of Chinese herbal medicines were randomly selected fromoutpatient clinics in 2018 for statistics.The medication situation and unreasonable prescription typeswere analyzed.Results The number of unreasonable prescriptions of Chinese herbal decoction pieces in outpatient clinics of our hospital in 2018 were 62,67,61 and 57 respectively,and their unreasonable rates were 4.13%,4.47%,4.07%and 3.80%,respectively.There are247(16.47%)unreasonable prescriptions out of 1500 prescriptions of Chinese herbal medicines,including repeated medications(19.84%),overdose medications(14.17%),irregular usage(25.10%),compatibility taboos(16.60%),and medication inconsistency(13.36%)and irregular writing(10.93%).Conclusion The incidence of unreasonable use of Chinese herbal medicine prescriptions in outpatient clinics ofour hospital wasrelatively high.Multiple departments should beproposed to further strengthen cooperation in response to the use of medicines.Physicians need to standardize prescription writing,Chinese pharmacists should strengthen the review before adjustments and a reasonable reward and punishment system should be developed.The rational use of prescriptions of Chinese herbal medicines should be included in the performance evaluation to promote the rational use of medicines.
