摘要
【目的】明确老年卒中后癫痫患者中认知障碍发生率及老年卒中后癫痫患者认知障碍的危险因素。【方法】回顾性分析278例卒中后癫痫患者的基本资料,从患者的一般情况、卒中相关因素及癫痫相关因素三个方面探讨与认知障碍相关的危险因素,并采用多因素Logistic回归分析,明确其相关性。【结果】老年卒中后癫痫患者发生率为69.42%。年龄、高血压、糖尿病、活动减少、饮酒史、卒中部位为前循环供血区、卒中次数、皮层病灶、卒中手术史、生活能力评分(modified rankin scale,mRS)、发作频率、脑电图表现为背景节律减慢等因素作为影响老年卒中后癫痫患者认知功能障碍的危险因素,具有统计学意义(P<0.05)。多因素Logistic回归分析结果发现高龄、前循环区域卒中、多次卒中、皮层病灶、癫痫频繁发作、脑电图背景节律慢化、高mRS评分可能与老年卒中后癫痫患者认知障碍的发生相关。【结论】高龄、癫痫频繁发作、多次卒中、前循环区域卒中、皮层病灶、脑电图背景节律慢化、高mRS评分可作为老年卒中后癫痫患者认知障碍的独立危险因素,提示临床医师在诊疗过程中,应注意高危人群的管控。
【Objective】To determine the incidence of cognitive impairment in elderly patients with post-stroke epilepsy,and explore its risk factors.【Methods】The basic data of 278 patients with post-stroke epilepsy were retrospectively analyzed,and the risk factors were discussed from three aspects as follows,patients’general situation,stroke related factors and epilepsy related factors.Multivariate logistic regression analysis was used to clarify the correlation.【Results】The incidence of epilepsy after stroke was 69.42%.Age,hypertension,diabetes,decreased activity,history of drinking,stroke in anterior circulatory area of blood supply,the number of stroke,cortical lesions,surgical history,modified Rankin Scale(mRS),seizure frequency,slowing down of background rhythm in electroencephalogram(EEG)were risk factors in patients with epilepsy after stroke with statistical significance(P<0.05).The results of multivariate logistic regression analysis showed that advanced age,anterior circulatory stroke,multiple strokes,cortical lesions,frequent seizures,slowing of background rhythm in EEG and high mRS score might be related to the occurrence of cognitive impairment in elderly patients with epilepsy after stroke.【Conclusion】Advanced age,frequent epileptic seizures,multiple strokes,anterior circulatory stroke,cortical lesions,background rhythm retardation in EEG and high mRS score can be independent risk factors for cognitive impairment in elderly patients with post-stroke epilepsy.In the process of diagnosis and treatment,clinicians should pay attention to the management and control of the high-risk population.
