摘要
目的探讨不同运动强度对经皮冠状动脉介入治疗(PCI)术后患者运动耐力的影响。方法选取2016年9月至2018年8月收治的73例PCI术后患者随机分为3组,高强度(通过心肺运动实验得出峰值功率的70%)有氧运动组(23例)、中等强度(通过心肺运动实验得出峰值功率的50%)有氧运动组(26例)、对照组(24例)。3组在严密的心电监护下分别按照不同的功率进行踏车训练,30 min/次,5次/周(或≥3次/周)及超声心动图。12周后再次进行心肺运动试验评估及超声心动图,2次评估结果比较心肺功能及运动耐力情况。30 min有氧运动包括静息5 min+无功率的踏车热身3 min+有负荷踏车训练(8~10 min达到目标负荷,然后保持目标负荷持续踏车训练)+恢复期5 min。结果训练前3组患者的峰值摄氧量(VO2peak)、峰值能量代谢当量(METpeak)、无氧阈(AT)、每分峰值通气量(VEpeak)、峰值心率(HRpeak)、左心室射血分数(LVEF)差异无统计学意义(P>0.05);训练后70%运动强度功率组及50%运动强度功率组峰值摄氧量(VO2peak)、峰值能量代谢当量(METpeak)、每分峰值通气量(VEpeak)、峰值心率(HRpeak)比较差异有统计学意义(P<0.05),且均优于对照组(P<0.05)。中等强度组与对照组LVEF比较差异无统计学意义(P>0.05),但均与高强度运动组差异有统计学意义(P<0.05);AT仅高强度运动组比对照组明显增高(P<0.05)。结论高强度运动处方可以更好地提高PCI术后患者心肺功能及运动耐力。
Objective To investigate the effects of different exercise intensities on exercise tolerance of patients after percutaneous coronary intervention(PCI).Methods Seventy-three patients who were treated by PCI were randomly divided into three groups,with high intensity(70%of peak power by cardiopulmonary exercise test)aerobic exercise group(n=23),moderate intensity(50%of peak power by cardiopulmonary exercise test)aerobic exercise group(n=26)and control group(24 cases).The patients in the three groups underwent treadmill training according to different powers under strict ECG monitoring,30 minutes once,5 times a week(not less than 3 times per week)and who were examined by echocardiography.After 12 weeks,the cardiac and lung exercise test evaluation and echocardiography were performed again.The results of the two evaluations were analyzed to compare the cardiopulmonary function and exercise endurance of patients.The 30-minute aerobic exercise included the resting 5 minutes+no power treadmill warm-up for 3 minutes+load treadmill training(8~10 minutes to reach the target load,then keep the target load for continuous treadmill training)+recovery period of 5 minutes.Results Before the training there were no significant differences in the peak oxygen uptake(VO2peak),peak energy metabolic equivalent(METpeak),anaerobic threshold(AT),peak ventilation per vent(VEpeak),maximum heart rate(HRpeak),left ventricular ejection and the scores(LVEF)among the three groups(P>0.05).After the training there were significant differences in the peak oxygen uptake(VO2peak),peak energy metabolic equivalent(METpeak),peak ventilation per minute(VEpeak)and maximum heart rate(HRpeak)between 70%exercise intensity power group and 50%exercise intensity power group(P<0.05).There were no significant differences in LVEF scores between moderate-intensity exercise group and control group(P>0.05),however,there were significant differences in LVEF scores between high-intensity exercise group and moderate-intensity exercise group and between high-intensity e
