摘要
目的观察脉冲冲洗联合负压封闭引流治疗颈椎后路椎板减压术后慢性感染的临床效果。方法选取北部战区总医院自2016年6月至2018年5月收治的27例颈椎后路椎板减压术后慢性感染患者为研究对象,所有患者在手术清创后应用脉冲冲洗联合负压封闭引流治疗慢性感染,观察治疗效果。结果27例患者术后均创面愈合,且获得6~12个月随访。其中,26例患者恢复良好,术后无感染、无复发;1例患者术后5 d切口上方红肿,局部拆线后流出约5.0 ml分泌物,并排出深部线节,向深部形成一直径0.8 cm、深度3.0 cm的窦道,细菌培养提示无细菌生长,经换药治疗2周后愈合,治疗期间间断排出多个深部线结,随访1年无复发。结论脉冲冲洗联合负压封闭引流治疗颈椎后路椎板减压术后慢性感染临床效果佳,可有效控制感染,避免创面长期迁延不愈,促进伤口愈合。
Objective To observe the clinical effect of pulse irrigation combined with vacuum sealing drainage on chronic infection after decompression of posterior cervical lamina.Methods A retrospective study was performed on 27 cases of patients with chronic infection after posterior cervical lamina decompression who were admitted from June 2016 to May 2018.All patients were treated with pulse irrigation combined with vacuum sealing drainage after surgical debridement for chronic infection,and the therapeutic effect was observed.Results All the 27 patients were followed up for 6 to 12 months.Among them,26 patients recovered well,no infection and recurrence after operation;1 patient had redness and swelling above the incision 5 days after operation,about 5.0 ml secretion flowed out after local suture removal,and discharged deep suture node,forming a diameter of 0.8 cm and depth of 3.0 cm sinuses,bacterial culture showed no bacterial growth,and the sinuses healed after 2 weeks of dressing change.During the treatment,several deep linear nodes were discharged intermittently,and there was no recurrence after 1 year of follow-up.Conclusion Pulse irrigation combined with vacuum sealing drainage for the treatment of chronic infection after posterior cervical lamina decompression has a good clinical effect,which can effectively control the infection,avoid the long-term delay of wound healing,and promote wound healing.
