摘要
目的:探讨不同分子分型乳腺癌患者腋窝淋巴结转移状况及其临床意义。方法:纳入2013年7月-2014年7月在我院行乳腺癌手术的195例女性患者的临床资料,根据患者免疫组化和(或)HER-2荧光原位杂交(FISH)结果将患者分为Luminal A型,Luminal B型,HER-2过表达型(ER-/PR-/HER-2+;ER+/PR-/HER-2+)和三阴性型并对比分析其腋窝淋巴结转移状况、病理参数及其预后因素。结果:Luminal B型,HER-2过表达型组腋窝淋巴结阳性率高于Luminal A型和三阴性型,其肿瘤大小与腋窝淋巴结转移率相关。Luminal A组预后优于HER-2+(ER+)组、HER-2+(ER-)组、三阴性型组,Luminal B组优于三阴性型组,差异有统计学意义(P<0.05)。结论:Luminal B型,HER-2过表达型组腋窝淋巴结阳性率较高;患者肿瘤大小与腋窝淋巴结转移率相关,Luminal A组预后相对较好,三阴性型及HER-2过表达型预后相对较差。
Objective:To investigate the status and clinical significance of axillary lymph node metastasis in breast cancer patients with different molecular typing.Methods:The clinical data of 195 women who underwent breast cancer surgery in our hospital from July 2013 to July 2014 were included.According to the results of immunohistochemistry and/or HER-2 fluorescence in situ hybridization(FISH),the patients were divided into Luminal A,Luminal B,HER-2 overexpression(ER-/PR-/HER-2+;ER+/PR-/HER-2+)and triple negative types.The status of axillary lymph node metastasis,pathological parameters and prognostic factors were analyzed.Results:The positive rate of axillary lymph nodes in Luminal B and HER-2 overexpression groups was higher than that in Luminal A and triple negative groups.The size of tumors was related to the rate of axillary lymph node metastasis.The prognosis of LuminalA group was better than that of HER-2+(ER+),HER-2+(ER-)and tri-negative group,and LuminalB group was better than that of tri-negative group.The difference was statistically significant(P<0.05).Conclusions:Luminal B and HER-2 overexpression groups had higher positive rate of axillary lymph nodes.The size of tumors was related to the rate of axillary lymph node metastasis.LuminalA group had better prognosis,tri-negative group and HER-2 overexpression group had higher positive rate of axillary lymph node metastasis.The latter is relatively poor.
