摘要
目的探讨卵巢交界性肿瘤行保留生育功能手术的临床意义。方法选取2015年1月~2016年5月在我院进行保留生育功能手术的卵巢交界性肿瘤患者30例,对患者的临床症状、手术方式进行记录分析,并在出院后走访患者术后病情复发、月经恢复、妊娠情况以及患者的满意度。结果保留生育能力手术的复发率为20%,妊娠率为70%,并且患者在手术后月经基本恢复正常。对于卵巢交界性肿瘤患者,相对于单侧肿瘤,采用肿瘤剥除和附件切除两种手术方式,其术后的复发率和妊娠率差异无统计学意义(P>0.05);对于双侧肿瘤,采用双侧肿瘤剥除与单侧附件切除+对侧肿瘤剥除,其妊娠率差异无统计学意义(P>0.05),其复发率差异有统计学意义(P<0.05)。结论对年轻的卵巢交界性肿瘤患者保留生育能力手术,虽然会提高病情的复发率,但对患者的妊娠率以及生活质量都有很大的提高,总的来说具有极高的临床价值。
Objective To explore the clinical significance of fertility-preserving surgery in the treatment of borderline ovarian tumors.Methods Thirty patients with borderline ovarian tumor who underwent fertility-preserving surgery in our hospital from January 2015 to May 2016 were selected.The patients'clinical symptoms and surgical methods were recorded and analyzed.After discharge,patients were followed up for postoperative recurrence,menstrual recovery,pregnancy,and their satisfaction.Results The recurrence rate and pregnancy rate of fertilitypreserving surgery was 20%and 70%respectively,and the patients'menstruation returned to normal after the surgery.In patients with borderline ovarian tumors,for unilateral tumors,there was no significant difference in postoperative recurrence rate and pregnancy rate between tumor removal and adnexectomy(P>0.05).For bilateral tumors,there was no significant difference in pregnancy rate between bilateral tumor removal and unilateral adnexectomy+contralateral tumor removal(P>0.05),but the difference in recurrence rate was statistically significant(P<0.05).Conclusion For fertility-preserving surgery in young patients with borderline ovarian tumors,although the recurrence rate of the disease will be increased,the pregnancy rate and quality of life of the patients will be greatly improved.On the whole,it has significant clinical values.
