摘要
目的研究神经刺激仪引导下腋路联合肌间沟臂丛神经阻滞在前臂手术中的应用效果。方法选取2018年10月~2019年5月本院收治的60例前臂手术患者作为研究对象,采用信封法随机将其分为对照组30例、观察组30例,对照组采用多点腋路臂丛神经阻滞,观察组采用神经刺激仪引导下腋路联合肌间沟臂丛神经阻滞。比较两组的麻醉效果、阻滞情况、运动及感觉阻滞评分、并发症发生情况。结果观察组前臂手术患者的麻醉优良率、首次阻滞成功率均高于对照组,差异有统计学意义(P <0.05),观察组的臂丛神经阻滞时间、手术等待时间短于对照组,差异有统计学意义(P <0.05);观察组不同神经的感觉阻滞评分、运动阻滞评分均高于对照组,差异有统计学意义(P <0.05);两组的并发症发生率进行比较,差异无统计学意义(P> 0.05)。结论在前臂手术中采用神经刺激仪引导下腋路联合肌间沟臂丛神经阻滞具有较好的麻醉阻滞效果,且安全性较高。
Objective To investigate the effect of neurostimulator-guided axillary and intermuscular groove brachial plexus block in patients undergoing forearm surgery. Methods 60 patients who underwent forearm surgery in our hospital from October 2018 to May 2019 were randomly divided into control group(30 cases)and observation group(30 cases)according to envelope method.The control group was treated with multi-point axillary brachial plexus block,while the observation group was treated with neurostimulator-guided axillary and intermuscular groove brachial plexus block.The anesthetic effects,block conditions,motor and sensory block scores and complications were compared between the two groups. Results The excellent rate of anesthesia and the success rate of the first block in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P < 0.05).The brachial plexus block time and operation waiting time in the observation group were shorter than those in the control group,and the differences were statistically significant(P < 0.05).The sensory block scores and motor block scores of different nerves in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P < 0.05).There was no significant difference in the incidence of complications between the two groups(P > 0.05). Conclusion Application of neurostimulator-guided axillary and intermuscular groove brachial plexus block in patients undergoing forearm surgery has better anesthesia block effect and higher safety.
