摘要
目的探讨溶藻弧菌的鉴定方法和生物学特性,为溶藻弧菌感染的诊断提供依据。方法对1例慢性外耳道炎患者耳部脓液标本分离的病原菌通过基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)、API细菌生化反应鉴定卡、Vitek 2 Compact鉴定仪和16S rRNA基因测序进行鉴定,采用抗菌药物敏感性试验检测耐药表型。结果该菌株鉴定为溶藻弧菌,药敏试验显示其对哌拉西林、氨苄西林/舒巴坦、哌拉西林/他唑巴坦、头孢他啶、头孢吡肟、亚胺培南、美罗培南、庆大霉素、阿米卡星、左氧氟沙星、环丙沙星和甲氧苄啶/磺胺甲噁唑敏感,对氨苄西林耐药,与患者的临床诊断及疾病预后相符合。结论 MALDI-TOF MS和16S rRNA基因测序对溶藻弧菌的鉴定准确可靠,对溶藻弧菌感染诊断具有重要价值,可供临床参考。
Objective To examine the methods of identifying Vibrio alginolyticus and its biological characteristics in order to provide a basis for the diagnosis of V.alginolyticus—associated infections.Methods The pathogenic bacteria isolated from ear pus specimens from a patient with chronic otitis externa were identified using matrix—assisted laser desorption/ionization—time of flight mass spectrometry(MALDI-TOF MS),an API test strip based on a bacterial biochemical reaction,the Vitek 2 Compact system,and 16S rRNA gene sequencing.In addition,the antibiotic—resistant phenotype of the bacterial strain was analyzed using antibiotic susceptibility testing.Results The strain was identified as V.alginolyticus,and antibiotic susceptibility testing indicated that it was sensitive to piperacillin,ampicillin/sulbactam,piperacillin/tazobactam,ceftazidime,cefepime,imipenem,meropenem,gentamicin,amikacin,levofloxacin,ciprofloxacin,and trimethoprim—sulfamethoxazole but resistant to ampicillin,all of which were consistent with the patient's clinical diagnosis and prognosis.Conclusion With the aid of MALDI-TOF MS and 16S rRNA gene sequencing,V.alginolyticus was accurately and reliably identified.This case is important for the diagnosis of V.alginolyticus—associated infections,and it provides a reference for clinicians.
