不同洗涤液的自体输血在失血性休克患者手术中的应用比较 被引量:1

摘要 目的:探讨不同洗涤液的自体输血在失血性休克患者手术中的应用效果。方法:收集我院2016年8月-2018年2月收治的60例失血性休克急诊手术患者作为研究对象,60例患者均应用自体输血,应用抽签法将60例患者分为生理盐水组、乳酸林格氏液组,醋酸林格氏液组,每组各20例。比较三组患者自体血回输前、后2 h的pH酸碱度、血乳酸(Lac)、血Na+、血Cl-、血K+、血红蛋白(HGB)、血小板(PLT)水平及凝血功能,并统计三组患者输血不良反应发生情况。结果:三组会输血前的上述指标比较差异均无统计学意义(P>0.05),醋酸林格氏液组回输后2 h的pH酸碱度、Lac均较其他两组低(P<0.05),其他指标与其他两组比较无明显差异(P>0.05)。结论:自体血生理盐水、乳酸林格氏液、醋酸林格氏液洗涤均能获得良好效果,但醋酸林格氏液洗涤能够降低患者血pH酸碱度和Lac,在维持患者体内酸碱平衡方面的作用更明显。 Objective:To explore the effect of autologous blood transfusion of different detergents in patients with hemorrhagic shock.Methods:60 patients admitted to our hospital for emergency surgery for hemorrhagic shock from August 2016 to February 2018 were collected as the study subjects.All 60 patients were treated with autotransfusion,and the 60 patients were divided into normal saline group,lactate ringer's solution group,and acetate ringer's solution group,with 20 patients in each group.PH,Lac,Na+,Cl-,K+,hemoglobin(HGB),platelet(PLT)and coagulation function of the three groups of patients before and after autotransfusion were compared.Results:There was no statistically significant difference in the above indexes between the three groups before transfusion(P>0.05),the pH value and Lac of ringer's acetate solution group at 2h after transfusion were lower than those of the other two groups(P<0.05),and the other indexes were not significantly different from those of the other two groups(P>0.05).Conclusion:Autologous blood saline,ringer's lactate solution and ringer's acetate solution can all achieve good results,but ringer's acetate solution can reduce the patients'blood pH value and Lac,which is more significant in maintaining the acid-base balance in patients.
