摘要
目的探讨胃癌根治术前实施强化沟通的效果及对患者术后免疫功能的影响。方法选择2017年3月至2018年3月延安市中医医院、延安大学附属医院接收并诊治的胃癌患者64例为研究对象,采用随机数字法将其分为参照组和强化沟通组,各32例。参照组患者采用常规护理干预,强化沟通组患者采用术前强化沟通护理干预。比较两组患者护理前、护理2周后的心理压力、情绪状态、免疫功能指标水平及生命质量。结果护理前,两组患者的心理压力、情绪状态、免疫功能指标水平及生命质量比较,差异无统计学意义(P>0.05)。护理2周后,两组患者的心理压力及情绪状态评分均明显低于护理前,且强化沟通组均明显低于参照组(P<0.05);护理2周后,两组患者的CD4+、CD3+水平均明显低于护理前,CD8+水平均明显高于护理前,且强化沟通组明显优于参照组(P<0.05);护理2周后,两组患者的功能领域、总体健康状况领域评分均明显高于护理前,症状领域评分均明显低于护理前,且强化沟通组优于参照组(P<0.05)。结论胃癌根治术前实施强化沟通的效果显著,可缓解患者的心理压力,改善免疫功能,提高生命质量,可作为胃癌患者的首选护理方法。
Objective To investigate the effect of intensive communication before radical operation of gastric cancer and its influence on postoperative immune function.Methods From March 2017 to March 2018,64 patients with gastric cancer treated in Yan’an city hospital of TCM and Yan’an university affiliated hospital were selected as the study objects,and the patients were divided into reference group and intensive communication group according to the ramdon number method,with 32 cases in each group.The patients in the reference group received routine nursing intervention,while the patients in the intensive communication group received preoperative intensive communication nursing intervention.The psychological stress,emotional state,levels of immune function indexes and quality of life before and after 2 weeks of nursing in the two groups were compared.Results Before nursing,there were no significant differences in psychological stress,emotional state,levels of immune function indexes and quality of life between the two groups(P>0.05).After 2 weeks of nursing,the scores of psychological stress and emotional state in the two groups were significantly lower than those before nursing,and those in the intensive communication group were significantly lower than the reference group(P<0.05);after 2 weeks of nursing,the levels of CD4^+and CD3^+in the two groups were significantly lower than those before nursing,and the leve of CD8^+was significantly higher than that before nursing,and those in the intensive communication group were significantly better than the reference group(P<0.05);after 2 weeks of nursing,the scores of functional areas and overall health status areas of the two groups were significantly higher than those before nursing,the scores of symptom areas were significantly lower than those before nursing,and those in the intensive communication group were better than the reference group(P<0.05).Conclusion The intensive communication before radical operation of gastric cancer has significant effect,it can relieve th
