外周血循环肿瘤细胞检测在非小细胞肺癌治疗的疗效判断及预后预测中的作用 被引量:1

摘要 目的探究外周血循环肿瘤细胞(CTCs)的检测在非小细胞肺癌(NSCLC)治疗的疗效判断及预后预测中的作用。方法收集本院收治的NSCLC患者64例,检测其治疗前、治疗后3周及3个月的外周血CTCs数量,患者均随访1年,对其局部转移复发率进行统计。探讨CTCs及其变化与NSCLC疗效、预后之间的关系。结果 CTCs阴性患者的TNM总分期、是否远处转移的构成比与CTCs阳性患者有显著差异(P<0.05)。治疗后3周及治疗后3个月,NSCLC患者的CTCs数目及阳性率均低于治疗前(P<0.05)。治疗后3个月,CTCs阴性患者的RR高于CTCs阳性患者(P<0.05)。治疗后随访期间,CTCs阴性患者局部转移复发率低于CTCs阳性患者(P<0.05)。结论外周血CTCs检测可用于NSCLC的疗效评价,且对患者具有一定的预后预测价值。 Objective To explore the role of detection of circulating tumor cells(CTCs) in peripheral blood in judgment of therapeutic effect and prognosis forecast of non-small cell lung cancer(NSCLC) treatment.Methods A total of 64 NSCLC patients in our hospital were collected.The number of peripheral blood CTCs were measured before treatment,at 3 weeks and 3 months after treatment.The patients were followed up for 1 year,and the recurrence rate of local metastasis in NSCLC patients was statistically analyzed.The relationship between CTCs and its changes and the efficacy,prognosis of NSCLC were explored.Results The composition ratio of total TNM stage and distant metastasis or not in CTCs negative patients were significantly different from those in CTCs positive patients(P<0.05).At 3 weeks and 3 months after treatment,the number and positive rate of CTCs in NSCLC patients were lower than those before treatment(P<0.05).At 3 months after treatment,the RR of CTCs negative patients was higher than that of CTCs positive patients(P<0.05).During the follow-up after treatment,the local metastatic recurrence rate of CTCs negative patients was lower than that of CTCs positive patients(P<0.05).Conclusion The detection of CTCs in peripheral blood can be used to evaluate the curative effect of NSCLC,and it has a certain forecast value of prognosis.
