摘要
目的探讨胃泌素17(G17)及胃蛋白酶原(PG)Ⅰ、Ⅱ检测在消化性溃疡中的诊断价值。方法选取2017年11月-2019年5月于大连大学附属中山医院消化内科就诊并行胃镜检查符合入选标准的75例患者。根据胃镜及结果分为健康对照组30例,良性溃疡组35例,恶性溃疡组10例。抽取空腹外周静脉血5ml,离心后应用酶联免疫吸附法测定G17、PGⅠ、PGⅡ,并计算PGⅠ与PGⅡ比值PGR。分析比较各组血清学指标的水平变化。结果溃疡组血清PGⅡ和G17水平显著高于对照组(P <0.05)。溃疡组的幽门螺杆菌阳性率显著高于对照组(P <0.001)。消化性溃疡不同亚组和对照组之间的血清PGⅡ和G17水平差异有统计学意义(P <0.05)。恶性溃疡组的PGⅡ水平显著高于良性溃疡组和对照组(P <0.05)。良性溃疡组G17水平显著高于恶性溃疡组和对照组(P <0.05)。幽门螺杆菌阳性率从高到低依次为恶性溃疡组、良性溃疡组和对照组(P <0.05)。血清PGⅠ、PGⅡ及幽门螺旋杆菌感染是感染良恶性溃疡的风险因素(P <0.05),血清G17是患恶性溃疡的风险因素(P <0.05)。结论血清胃蛋白酶原Ⅱ和胃泌素17对判断消化性溃疡的溃疡性质及制定治疗方案有一定的指导意义。随着病情加重(对照组-良性溃疡组-恶性溃疡组),感染幽门螺杆菌的概率增高。血清PGⅡ、G17是患恶性溃疡的独立影响因素。血清G17对消化性溃疡的诊断效率最高,敏感度性最佳,血清PGⅡ特异度更高。
Objective To investigate the diagnostic value of gastrin 17(G17)and pepsinogen(PG)ⅠandⅡ detection in peptic ulcer. Methods Seventy-five patients who met the enrollment criteria and received gastroscopy at the Department of Gastroenterology,Zhongshan Hospital,Dalian University from November 2017 to May 2019 were collected.According to gastroscopy results,they were divided into a healthy control group(n=30),a benign ulcer group(n =35)and a malignant ulcer group(n =10).G17,PGⅠand PGⅡwere measured by ELISA,and the ratio of PGⅠto PGⅡ(PGR)was calculated.The changes of serological index levels were analyzed and compared between every group. Results The levels of serum PGⅡand G17 in the ulcer group were significantly higher than those in the control group(P <0.05).The positive rate of Helicobacter pylori in the ulcer group was significantly higher than that in the control group(P <0.001).There were significant differences in serum PGⅡand G17 levels between different subgroups of peptic ulcer and the control group(P <0.05).The level of PGⅡin the malignant ulcer group was significantly higher than that in the benign ulcer group and the control group(P <0.05).The level of G17 in benign ulcer group was significantly higher than that in malignant ulcer group and control group(P <0.05).The positive rate of Helicobacter pylori from high to low ranged as malignant ulcer group,benign ulcer group and control group(P <0.05).Serum PGⅠ,PGⅡand Helicobacter pylori infection were risk factors for benign and malignant ulcer infection(P <0.05),and serum G17 was a risk factor for malignant ulcer(P <0.05). Conclusion Serum pepsinogen Ⅱand gastrin 17 have certain guiding significance for judging the ulcer properties of peptic ulcer and formulating a treatment plan.As the disease worsens(control group-benign ulcer group-malignant ulcer group),the probability of infection with Helicobacter pylori is higher.Serum PGⅡ and G17 are independent influencing factors for malignant ulcers.Serum G17 has the highest diagnostic e
