摘要
目的探讨老年呼吸机相关性肺炎患者炎症因子、乳酸、和肽素及免疫功能的水平变化。方法选取2017年1月-2018年6月本院重症医学科收治的88例老年呼吸机相关性肺炎患者作为研究对象,根据预后分为存活组(n=56)和死亡组(n=32),另选取50例健康志愿者作为对照组。检测并比较各组对象的降钙素原(PCT)、C-反应蛋白(CRP)、白细胞介素-8(IL-8)、白介素-10(IL-10)、乳酸(LACT)、和肽素及T细胞亚群。结果死亡组的PCT、CRP、IL-8、LACT、和肽素水平高于存活组与对照组,存活组的PCT、CRP、IL-8、LACT、和肽素水平高于对照组,差异均有统计学意义(P<0.05);死亡组的IL-10、CD3^+、CD4^+、CD4^+/CD8^+水平低于存活组与对照组,存活组的IL-10、CD3^+、CD4^+、CD4^+/CD8^+水平低于对照组,差异均有统计学意义(P<0.05)。结论老年呼吸机相关性肺炎死亡组的炎症因子、乳酸、和肽素及免疫功能最不理想,监测上述指标可能为评估预后提供一定参考依据。
Objective To investigate the changes of inflammatory factors,lactic acid,peptide and immune function in elderly patients with ventilator-associated pneumonia.Methods A total of 88 elderly patients with ventilator-associated pneumonia admitted to the Department of Critical Care Medicine of our hospital from January 2017 to June 2018 were selected as the study subjects.According to the prognosis,they were divided into survival group(n=56)and death group(n=32).50 healthy volunteers were selected as control group.Procalcitonin(PCT),C-reactive protein(CRP),interleukin-8(IL-8),interleukin-10(IL-10),lactic acid(LACT),peptide and T cell subsets were detected and compared.Results The levels of PCT,CRP,IL-8,LACT and peptide in the death group were higher than those in the survival group and the control group.The levels of PCT,CRP,IL-8 LACT and peptide in the survival group were higher than those in the control group,with the differences statistically significant(P<0.05).The levels of IL-10,CD3^+,CD4^+,CD4^+/CD8^+in the death group were lower than those in the survival group and the control group.The levels of IL-10,CD3^+,CD4^+,CD4^+/CD8^+in the survival group were lower than those in the control group,with the differences of statistical significance(P<0.05).Conclusion Inflammatory factors,lactic acid,peptide and immune function were the most unsatisfactory in elderly patients with ventilator-associated pneumonia.Monitoring the above indicators may provide some reference for evaluating prognosis.
