摘要
目的探讨胰岛β细胞自身抗体联合检测对糖尿病分型的诊断价值。方法收集2016年4月-2018年10月收治的糖尿病患者120例,将50例Ⅰ型糖尿病纳入A组,将70例Ⅱ型糖尿病纳入B组,另外选取体检的健康者60例纳入对照组。测定三组血清谷氨酸脱羧酶抗体(GADA)、胰岛细胞抗体(ICA)、胰岛素自身抗体(IAA)水平,分析GADA、ICA及IAA单独及联合诊断糖尿病分型的效能。结果A组GADA、ICA、IAA单独及联合阳性检出率高于B组、对照组,且B组GADA、ICA、IAA单独及联合阳性检出率高于对照组,差异具有统计学意义(P<0.05);GADA诊断Ⅰ型糖尿病的ROC曲线下面积AUC为0.729,灵敏度为0.667,特异度为0.815;GADA的AUC为0.711,灵敏度为0.652,特异度为0.807;IAA的AUC为0.716,灵敏度为0.638,特异度为0.811;联合诊断的AUC为0.851,灵敏度为0.820,特异度为0.932。GADA诊断Ⅱ型糖尿病的ROC曲线下面积AUC为0.718,灵敏度为0.578,特异度为0.805;GADA的AUC为0.709,灵敏度为0.592,特异度为0.813;IAA的AUC为0.710,灵敏度为0.587,特异度为0.795;联合诊断的AUC为0.844,灵敏度为0.817,特异度为0.952。结论早期联合诊断GADA、ICA、IAA对糖尿病分型具有重要诊断价值。
Objective To investigate the diagnostic value of joint detection of islet β cell autoantibodies in classification of diabetes.Methods 120 patients with diabetes who admitted in the hospital from April 2016 to October 2018 were retrospectively selected.50 patients with type Ⅰ diabetes were included in A group,70 patients with typeⅡdiabetes were included in B group,and 60 healthy people with physical examination during the same time period were included in control group.The levels of GADA,ICA and IAA in the three groups were detected.The efficiency of single GADA,ICA,IAA and joint detection in diagnosis of classification of diabetes was analyzed.Results The positive detection rate of single GADA,ICA,IAA and joint detection in A group was higher than that in B group and control group,the positive detection rate of single GADA,ICA,IAA and joint detection in B group was higher than that in control group(P<0.05);The area under the ROC curve of GADA in diagnosis of typeⅠdiabetes was 0.729,the sensitivity was 0.667,and the specificity was 0.815;The AUC of GADA was 0.711,the sensitivity was 0.652,and the specificity was0.807.The AUC of IAA was 0.716,the sensitivity was 0.638,and the specificity was 0.811;The AUC of joint diagnosis was 0.851,the sensitivity was 0.820,and the specificity was 0.932.The area under the ROC curve of GADA in diagnosis of typeⅡdiabetes was 0.718,the sensitivity was 0.578,the specificity was 0.805;The AUC of GADA was 0.709,the sensitivity was 0.592,and the specificity was 0.813;The AUC of IAA was 0.710,the sensitivity was0.587,and the specificity was 0.795;The AUC of joint diagnosis was 0.844,the sensitivity was 0.817,and the specificity was 0.952.Conclusions The early joint diagnosis of GADA,ICA and IAA has important diagnostic value in classification of diabetes.
