肺部感染对乳腺癌手术妇女TLR7、IL-23、IL-17信号通路的影响 被引量:4

Effect of pulmonary infection on TLR7/IL-23/IL-17 signaling pathway in women with breast cancer surgery

摘要 目的探讨肺部感染对乳腺癌手术妇女Toll样受体(Toll-like receptors,TLR7)、白细胞介素-23(Interleukin-23,IL-23)、白细胞介素-17(Interleukin-17,IL-17)信号通路的影响。方法回顾性分析2015年7月-2018年7月于内蒙古医科大学附属医院接受治疗的866例乳腺癌患者的临床资料,根据患者是否并发肺部感染分为并发感染组(n=304)和未并发感染组(n=562),采用酶联免疫吸附法检测患者感染前后血清白细胞介素-2(Interleukin-2,IL-2)、白细胞介素-6(Interleukin-6,IL-6)、白细胞介素-8(Interleukin-8,IL-8)、白细胞介素-10(Interleukin-10,IL-10)、白细胞介素-17(Interleukin-17,IL-17)、白细胞介素-23(Interleukin-23,IL-23)、C-反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)炎症因子水平变化情况。采用流式细胞仪检测血液T淋巴细胞4(T Cell response,CD4^+)、T淋巴细胞8(T Cell response,CD8^+),分析血液中免疫球蛋白A(Immunoglobulin A,IgA)、免疫球蛋白G(Immunoglobulin G,IgG)、免疫球蛋白M(Immunoglobulin M,IgM)免疫抗体含量。采用荧光定量检测法检测外周血TLR7 mRNA表达量。结果乳腺癌患者肺部感染后血清IL-2、IL-6、IL-8、IL-10、IL-17-IL-23、TNF-α、CRP指标水平分别高于感染前,差异具有统计学意义(P<0.05)。乳腺癌患者肺部感染前后CD4^+免疫细胞含量、IgA免疫抗体含量对比无统计学差异,乳腺癌患者肺部感染后CD8^+免疫细胞含量、IgG、IgM免疫抗体含量低于感染前,差异具有统计学意义(P<0.05)。乳腺癌患者肺部感染后单个核细胞TLR7 mRNA表达为(0.76±0.08),TLR7蛋白表达为(0.49±0.04),低于感染前单个核细胞TLR7 mRNA表达,TLR7蛋白表达,差异具有统计学意义(P<0.05)。结论乳腺癌患者发生肺部感染后血清IL-17、IL-23指标水平明显上升,且肺部感染抑制机体TLR7 mRNA表达量和蛋白表达量,TLR7表达量下降会促进乳腺癌患者IL-23/IL-17信号通路的活化程度。 OBJECTIVE To explore the effect of pulmonary infection on Toll-like receptor(TLR7),interleukin-23(IL-23),interleukin-17(IL-17)signaling pathway in breast cancer surgery women.METHODS Retrospective analysis of clinical data of 866 breast cancer patients,who were treated in the Affiliated Hospital of Inner Mongolia Medical University from July to May 2018 were recruited.They were divided into concurrent infection group(n=304)and non-concurrent group according to whether or not the patient was complicated with pulmonary infection.In the infected group(n=562),levels of serum interleukin-2(IL-2),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-10(IL-10),interleukin-17(IL-17),interleukin-23(IL-23),C-reactive protein(CRP)and Tumor necrosis factor-α(TNF-α)were detected by enzyme-linked immunosorbent assay(ELISA).Flow cytometry was used to detect the percentages of CD4^+T cells and CD8^+T cells.Antibody content of immunoglobulin A(IgA)and immunoglobulin G and Immunoglobulin M(IgM)in blood were detected by AutoLumo A1000.Fluorescence quantitative detection was used to detect mRNA expression of TLR7 in peripheral blood.RESULTS The levels of serum IL-2,IL-6,IL-8,IL-10,IL-17-IL-23,TNF-αand CRP in patients with breast cancer were significantly higher aftter the pulmonary infection than before(P<0.05).There was no significant difference of the content of CD4^+immune cells and IgA immune antibody between breast cancer patients before and after lung infection.The content of CD8^+immune cells,IgG and IgM in breast cancer patients after infection were significantly lower than that before infection(P<0.05).TLR7 mRNA expression in mononuclear cells of breast cancer patients was(0.76±0.08),and the protein expression was(0.49±0.04)after the pulmonary infection,which were significantly lower than that before infection,with TLR7 mRNA expression and protein expression(P<0.05).CONCLUSION The serum levels of IL-17 and IL-23 in patients with breast cancer increased significantly after lung infection,which inhibited TLR7 mRNA expressi
