摘要
目的了解2017年临床检出病原菌的耐药性,为指导临床合理使用抗菌药物提供理论依据。方法利用微生物分离鉴定系统及纸片扩散法(K-B法)等,对2017年医院门急诊和住院患者临床检出病原菌的分布情况及耐药性进行回顾性分析。结果2017年共分离病原菌4547株,其中革兰阴性菌3163株占69.56%,革兰阳性菌983株占21.62%,真菌401株占8.82%。检出前五位的病原菌分别为大肠埃希菌、肺炎克雷伯菌、鲍氏不动杆菌、金黄色葡萄球菌和铜绿假单胞菌。大肠埃希菌和肺炎克雷伯菌对碳青霉烯类抗菌药物的耐药率分别<3.00%和10.00%。铜绿假单胞菌对亚胺培南和美罗培南的耐药率分别为14.67%和6.00%;鲍氏不动杆菌对亚胺培南和美罗培南的耐药率分别高达78.88%和79.37%。结论2017年临床分离病原菌主要以革兰阴性菌为主,且革兰阴性菌的耐药形势非常严峻,合理选用抗菌药物具有重要意义。
OBJECTIVE To investigate the clinical distribution and drug resistance of clinically detected pathogens in 2017,so as to provide theoretical basis for clinical rational drug usage.METHODS The distribution and drug resistance of the clinically detected pathogens in patients of outpatient and inpatient departments in 2017 were retrospectively analyzed by using the microorganisms isolation and identification system and the disk diffusion method(K-B method).RESULTS A total of 4547 strains of pathogens were identified.There were 3163 strains of Gram-negative bacteria(69.56%),983 strains of Gram-positive bacteria(21.62%)and 401 strains of fungi(8.82%).The top five clinical pathogens were Escherichia coli(24.28%),Klebsiella pneumoniae(16.74%),Acinetobacter baumannii(9.06%),Staphylococcus aureus(8.69%)and Pseudomonas aeruginosa(6.60%).The drug resistance rates of E.coli and K.pneumoniae against hydrocarbon antibacterial agents were below 3.00%and 10.00%respectively.The drug resistance rates of P.aeruginosa against imipenem and meropenem were 14.67%and 6.00%respectively.The drug resistance rates of A.baumannii against imipenem and meropenem were as high as 78.88%and 79.37%respectively.CONCLUSION The Gram-negative bacteria were the major pathogens isolated in 2017,and the drug resistance of the Gram-negative bacteria was very serious.It is of great signigicance to choose antibiotics in a rational manner.
