摘要
目的探讨miR-146a和血管内皮生长因子(VEGF)在视网膜中央静脉阻塞(CRVO)伴发黄斑水肿患者房水中的表达及意义。方法选取2016年2月至2018年8月就诊的112例CRVO伴发黄斑水肿患者为研究对象,另选取同期60例无眼部疾病的白内障患者为对照组。患者入院后接受全面的眼科检查,玻璃体腔给药,用药前采集房水,qRT-PCR检测房水中miR-146a与VEGF表达水平,采用酶联免疫吸附法(ELISA)检测肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、VEGF水平,相关性分析采用Pearson相关系数法,Logistic分析影响CRVO伴发黄斑水肿病情的因素。结果2组年龄、性别、高血压比例差异均无统计学意义(P>0.05),ELISA检测发现研究组TNF-α、IL-6、VEGF水平显著高于对照组(P<0.05);qRT-PCR检测结果显示研究组中miR-146a(1.03±0.21)表达水平显著低于对照组miR-146a(7.93±1.51),研究组中VEGF mRNA(3.45±1.02)表达水平显著高于对照组VEGF mRNA(1.02±0.23),与ELISA检测结果一致;Pearson相关性分析显示miR-146a与VEGF呈显著负相关(r=-0.659,P=0.001),VEGF随miR-146a水平的降低而升高;Logistic多因素回归分析显示TNF-α、IL-6与VEGF水平均是影响CRVO伴发黄斑水肿病情的危险因素,miR-146a是CRVO伴发黄斑水肿病情的保护因素。结论miR-146a在CRVO伴发黄斑水肿患者中表达降低,VEGF在CRVO伴发黄斑水肿患者中表达升高,二者呈负相关,VEGF可能随着miR-146a表达的降低而升高进而影响CRVO伴发黄斑水肿病情的发生发展。
Objective To investigate the expression and significance of miR-146a and vascular endothelial growth factor(VEGF)in aqueous humor of patients with central retinal vein occlusion(CRVO)complicated by macular edema.Methods A total of 112 patients with CRVO complicated by macular edema who were treated in our hospital from February 2016 to August 2018 were enrolled research objects(research group),at the same time,the other 60 patients with cataract were enrolled as control group.The patients received comprehensive ophthalmologic examination after admission,and were intravitreally administrated.The aqueous humor before medication was collected,and the expression levels of miR-146a and VEGF in aqueous humor were detected by qRT-PCR.Moreover the levels of TNF-α,IL-6 and VEGF were detected by ELISA.The correlation analysis was performed by means of Pearson correlation coefficient method,and the factors affecting the condition of CRVO with macular edema was analyzed by Logistic analysis.Results There were no significant differences in patient's age and sex,and hypertension proportion between the two groups(P>0.05).ELISA detection showed that the levels of TNF-α,IL-6 and VEGF in research group were significantly higher than those in control group(P<0.05).The results of qRT-PCR showed that the expression levels of miR-146a were 1.03±0.21 in research group,which were significantly lower than those(7.93±1.51)in control group,and the expression levels of VEGF mRNA were 3.45±1.02,which were significantly higher than those(1.02±0.23)in control group,the results by qRT-PCR were consistent with those by ELISA.Pearson correlation analysis showed that miR-146a was negatively correlated with VEGF(r=0.659,P<0.01),and VEGF levels were increased with the decrease of miR-146a levels.Logistic multivariate regression analysis showed that TNF-α,IL-6 and VEGF were risk factors for CRVO with macular edema,miR-146a was a protective factor for CRVO with macular edema.Conclusion The expression levels of MiR-146a are decreased in patients
