PiCCO监测技术应用在严重脓毒症治疗中的临床指导作用分析 被引量:6

摘要 目的:探讨脉搏指示连续心排血量(PiCCO)监测技术应用在严重脓毒症治疗中的临床指导作用。方法:选取笔者所在医院2017年5月-2019年5月收治的严重脓毒症患者84例。根据不同监测方案将其分成两组,每组42例。比较对照组(常规监测)和观察组(PiCCO监测)的应用效果。结果:治疗前及治疗6、24 h两组PaO2、MAP对比差异无统计学意义(P>0.05),治疗12 h,观察组MAP、PaO2均高于对照组,差异均有统计学意义(P<0.05);观察组血管活性药物用量少于对照组,呼吸机使用时间和重症监护时间均短于对照组,急性肾损伤发生率(7.14%)低于对照组(28.57%),差异均有统计学意义(P<0.05)。结论:应用PiCCO监测技术能够指导严重脓毒症患者液体复苏治疗,减少用药量,降低急性肾脏损伤发生风险。 Objective:To explore the clinical guiding role of continuous cardiac output(PiCCO)monitoring technology in the treatment of severe sepsis.Method:A total of 84 patients with severe sepsis in our hospital from May 2017 to May 2019 were selected.According to different monitoring schemes,they were divided into two groups with 42 cases in each group.The application effects of control group(routine monitoring)and observation group(PiCCO monitoring)were compared.Result:There were no significant differences in PaO2,MAP between the two groups before treatment and 6 hours after treatment(P>0.05).At 12 hours of treatment,the MAP,PaO 2 of the observation group were higher than that of the control group,the differences were statistically significant(P<0.05).The dose of vasoactive agent in the observation group was less than that of the control group,the use time and the intensive care time of the ventilator were shorter than those of the control group,the incidence of acute renal injury(7.14%)was lower than that of the control group(28.57%),the differences were statistically significant(P<0.05).Conclusion:The application of PiCCO monitoring technology can guide the fluid resuscitation treatment of severe sepsis patients,reduce the dosage of drugs and reduce the risk of acute kidney injury.
