摘要
目的:观察祛痰熄风开窍汤联合针刺防治蛛网膜下腔出血后脑血管痉挛的临床疗效。方法:选取2016年8月~2017年8月入住我院神经内科的100例蛛网膜下腔出血后脑血管痉挛患者为研究对象,随机分为对照组和观察组,50例/组。对照组予以祛痰熄风开窍汤治疗,观察组在对照组基础上联用针刺治疗。采用酶联免疫吸附法(ELISA)检测治疗前后两组患者血清NF-kB及基质金属蛋白酶-9(MMP-9)水平;于治疗后3天、7天及14天检测患者大脑中动脉血流速度;比较两组患者的近期疗效及短期预后。结果:治疗后,两组患者血清NF-kB及MMP-9水平均显著降低(P<0.05),观察组血清NF-kB及MMP-9水平均显著低于对照组(P<0.05);治疗后,观察组大脑中动脉血流速度随时间延长显著升高,且在治疗后3天、7天及14天大脑中动脉血流速度显著高于对照组(P<0.05);观察组近期疗效及短期预后均优于对照组(P<0.05)。结论:祛痰熄风开窍汤联合针刺防治蛛网膜下腔出血后脑血管痉挛能显著降低患者大脑中动脉血流速度,减少脑水肿的产生,具有很好的近期疗效及短期预后。
Objective:To explore the clinical efficacy of Qutan Xifeng Kaiqiao Decoction combined with acupuncture in the prevention and treatment of cerebral vasospasm after subarachnoid hemorrhage. Methods:100 patients with cerebral vasospasm after subarachnoid hemorrhage in our department of neurology from August 2016 to August 2017 were selected for the study and were randomly divided into controlled group and observation group, with 50 cases in each group. The controlled group was treated with Qutan Xifeng Kaiqiao Decoction, and the observation group was treated with acupuncture on the basis of the controlled group. Serum levels of NF-kB and matrix metalloproteinase-9(MMP-9) were measured by enzyme-linked immunosorbent assay(ELISA) before and after treatment in the two groups. The blood flow velocity of middle cerebral artery was measured at 3 d, 7 d and 14 d after treatment. The short-term efficacy and short-term prognosis were compared between the two groups. Results:After treatment, the levels of serum NF-kB and MMP-9 were significantly decreased in the two groups(P<0.05), and the levels of serum NF-kB and MMP-9 in the observation group were significantly lower than those in the controlled group(P<0.05). After treatment, the blood flow velocity of middle cerebral artery in the observation group were increased significantly with time, and the blood flow velocity of middle cerebral artery was significantly higher than that in the controlled group at 3 d, 7 d and 14 d after treatment(P<0.05). The short-term efficacy and short-term prognosis in the observation group were better than those in the controlled group(P<0.05). Conclusion:Qutan Xifeng Kaiqiao Decoction combined with acupuncture can significantly reduce the blood flow velocity of middle cerebral artery and reduce the generation of cerebral edema in the prevention and treatment of cerebral vasospasm after subarachnoid hemorrhage, and it has good short-term efficacy and long-term efficacy.
