摘要
目的探讨应用医疗失效模式与效应分析(Healthcare Failure Mode and Effect Analysis,HFMEA)在血液透析患者置管感染控制中的效果。方法选取2016年1月至2017年12月行血液透析治疗的患者,按入院时间分为对照组和观察组,对照组采用血液透析常规护理措施,观察组应用HFMEA管理。比较应用HFMEA前后血液透析患者置管感染发生情况,失效模式的风险优先指数(Risk Priority Number,RPN)及患者满意度。结果应用HFMEA后血液透析患者置管感染事件明显减少;易造成患者感染的失效模式RPN值总分从2981分下降到1176分,总分下降了60.6%;患者满意度从80.35分上升到93.03分,差异具有统计学意义(t=24.964,p<0.05)。结论应用HFMEA可预见易导致血液透析置管感染的失效环节,并且通过RPN值将隐患得到量化,从而确定需要改进的重点环节,有效降低血液透析患者置管感染事件的发生进而提高患者满意度。
Objective To explore the effect of applying health-care failure mode and effect analysis(HFMEA)in catheter-related infection control for hemodialysis patients.Methods Hemodialysis patients admitted from January 2016 to December 2017 were recruited and divided into the control group and the observation group based on the admission time.The control group were given routine nursing services of hemodialysis,while the observation group received HFMEA care.The incidence of catheter infection,the risk priority number(RPN)of failure mode and patient satisfaction were compared before and after the HFMEA intervention.Results After the application of HFMEA,the incidence of catheter infections in hemodialysis patients was significantly reduced.The total RPN score that represented high-risk infections declined from 2981 to 1176,down by 60.6%.The score for patient satisfaction increased from 80.35 to 93.03.The difference was statistically significant(t=24.964,p<0.05).Conclusion The application of HFMEA can predict the failure links that could easily lead to catheter infections in hemodialysis and quantify the hidden risks through RPN value,thus determining the key links that need to be improved,effectively reducing the incidence of catheter infections in hemodialysis patients,and improving patient satisfaction.
