分级诊疗体系建设,让居民“回到社区”是关键。“1+1+1”家庭医生签约服务,其中重要服务功能之一就是延伸处方。广大的社区居民选择就近社区卫生服务中心签约,同时按照自身健康需求在全市范围内选择1家二级医院、1家三甲医院签约即“1+1+1”医疗机构组合签约服务。签约后居民享受预约、转诊、延伸处方等服务优惠。
