摘要
目的探讨老年骨质疏松性椎体压缩骨折采用高黏度骨水泥经皮椎体成形术(PVP)的治疗效果。方法选择本院2016年1月至2017年8月收治的老年骨质疏松性椎体压缩骨折患者104例作为研究对象,随机分为对照组和研究组各52例。对照组行低黏度水泥经皮椎体后凸成形术(PKP)治疗,研究组行高黏度骨水泥PVP术治疗,术后随访1年,比较两组手术相关指标、椎体高度和形态及VAS、ODI评分、骨水泥渗透率。结果研究组骨水泥用量、透视次数较对照组少,手术时间较对照组短,骨水泥渗透率较对照组低,差异有统计学意义(P<0.05);两组术后12个月椎体后凸Cobb角、VAS和ODI评分均较术前低,椎体前缘高度较术前高,差异有统计学意义(P<0.05);术后12个月两组椎体后凸Cobb角、椎体前缘高度、VAS和ODI评分相比,差异无统计学意义(P>0.05)。结论低黏度水泥PKP术和高黏度骨水泥PVP术均可有效治疗老年骨质疏松性椎体压缩骨折,恢复椎体高度,改善椎体形态,减轻术后疼痛,改善腰椎功能,而高黏度骨水泥PVP手术操作更为简单,可缩短手术时间,降低术后辐射量及骨水泥渗透率,安全性较高。
Objective To investigate the effects of percutaneous vertebroplasty(PVP)with high viscosity bone cement in treatment of senile osteoporotic vertebral compression fractures(OVCF).Methods A total of 104 senile patients with OVCF in the hospital from January 2016 to August 2017 were selected,and they were randomly divided into two groups,with 52 cases in each group.Control group was given percutaneous kyphoplasty(PKP)with low viscosity cement,while research group was treated with high viscosity bone cement PVP.After surgery and 1-year follow-up,the surgical indexes,vertebral height and form,VAS and ODI scores,bone cement leakage rate were compared between the two groups.Results The dosage of bone cement and times of fluoroscopy in research group were less than those in control group,the operation time of research group was shorter than control group,and the bone cement leakage rate of research group was lower than control group(P<0.05).After surgery for 12 months,the vertebral kyphotic Cobb angle,VAS and ODI scores in two groups were lower than those before surgery,while the anterior vertebral height was higher than that before surgery(P<0.05).After surgery for 12 months,there was no statistical difference in the vertebral kyphotic Cobb angle,anterior vertebral height,VAS and ODI scores between the two groups(P>0.05).Conclusion Low viscosity cement PKP and high viscosity bone cement PVP in treatment of senile OVCF are effective,they can recover vertebral height,improve vertebral form,reduce postoperative pain and improve lumbar function.However,high viscosity bone cement PVP has easier operation,it can shorten operation time,decrease postoperative radiant quantity and bone cement leakage rate,which has higher safety.
