摘要
目的探讨静脉溶栓急性脑梗死患者出院准备度及相关因素的分析。方法回顾性分析2013年1月至2018年6月出院的168例静脉溶栓急性脑梗死患者临床资料。采用出院准备度量表(RHDS)、社会支持量表(SSRS)、出院指导质量量表(QDTS)评估患者的出院准备度,采用Pearson相关分析法分析RHDS得分与SSRS、QDTS得分的相关性,并采用多元回归分析确定静脉溶栓急性脑梗死患者出院准备度的影响因素。结果Pearson相关分析显示:RHDS得分与SSRS、QDTS得分均呈正相关关系(P<0.05);单因素分析显示,不同性别、年龄的急性脑梗死患者RHDS得分比较,差异无统计学意义(P>0.05);文化程度高中及以上、已婚、与家人同住、家庭人均月收入≥3000元、由子女和配偶共同照顾、合并基础疾病少、用药种类少是出院准备度的影响因素(P<0.05);多元回归分析结果显示,经济状况、基础病史、SSRS得分、QDTS得分是影响RHDS得分的主要因素(P<0.05)。结论急性脑梗死患者出院准备度影响因素较多,主要为社会支持、出院指导质量、经济因素及基础病史。
Objective To explore the analysis of preparedness for discharge and related factors of patients with intravenous thrombolytic acute cerebral infarction.Methods To retrospectively analyze the clinical data of 168 cases of patients with intravenous thrombolytic acute cerebral infarction who discharged from January 2013 to June 2018.The readiness for hospital discharge scale(RHDS),social support rating scale(SSRS),and quality of discharge teaching scale(QDTS)were used to assess patients'readiness for discharge.A Pearson correlation analysis was used to analyze the correlation between RHDS scores and SSRS,QDTS scores,and multivariate regression analysis was used to determine the influencing factors of patients with intravenous thrombolytic acute cerebral infarction.Results Pearson correlation analysis showed that the RHDS scores were positively correlated with SSRS and QDTS scores(P<0.05).Univariate analysis showed that there was no significant difference in RHDS scores between patients with acute cerebral infarction of different genders and ages(P>0.05).The high school education or above,married,living with family,family average monthly Income≥RMB 3,000,taken care by children and spouse,less combined basic illnesses,and less medication types were the influencing factors of readiness for discharge(P<0.05).Multivariate regression analysis showed that the economic status,history of basic illnesses,SSRS score and QDTS score were the main factors affecting RHDS scores(P<0.05).Conclusion The patients with acute cerebral infarction had many factors influencing readiness for discharge,mainly including social support,quality of discharge guidance,economic factors and history of basic illnesses.
