摘要
目的研究慢性阻塞性肺疾病急性加重期(AECOPD)合并呼吸衰竭患者临床治疗中应用早期肠内营养(EN联合胸腺肽a1的效果。方法随机将2016年1月-2019年6月共65例AECOPD合并呼吸衰竭患者分为两组,对照组32例接受接受早期肠内营养治疗,观察组33例接受早期肠内营养联合胸腺肽a1治疗,比较两组治疗效果。结果观察组治疗1周后白细胞计数、超敏C反应蛋白水平均低于对照组,差异有统计学意义(P<0.05);观察组治疗1周后白蛋白、前白蛋白、血红蛋白、转铁蛋白水平均高于对照组,差异有统计学意义(P<0.05);观察组机械通气时间、ICU住院时间为(9.23±3.26)d、(11.46±3.66)d,对照组(14.51±3.61)d、(15.74±3.81)d,差异有统计学意义(t=6.193、4.619,P<0.05)。结论AECOPD合并呼吸衰竭接受早期肠内营养联合胸腺肽a1治疗对患者营养状况改善更明显,能够获得更高的临床整体疗效,值得推广。
Objective To study the effect of early enteral nutrition(EN)combined with thymosin a1 in the clinical treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with respiratory failure.Methods A total of 65 AECOPD patients with respiratory failure from January 2016 to June 2019 were randomLy divided into 2 groups.32 patients in the control group received early enteral nutrition therapy,and 33 patients in the observation group received early enteral nutrition combined with thymosin a1,comparing the effects of the two groups of treatment.Results After 1 week of treatment,the white blood cell count and high-sensitivity C-reactive protein levels in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).The albumin,prealbumin,hemoglobin and transferrin levels were higher in the observation group than 1 week after treatment of the treatment group,the difference was statistically significant(P<0.05);the mechanical ventilation time and ICU hospitalization time in the observation group were(9.23±3.26)d,(11.46±3.66)d,and the control group(14.51±3.61)d,(15.74±3.81)d,the difference was statistically significant(t=6.193,4.619,P<0.05).Conclusion AECOPD combined with respiratory failure with early enteral nutrition combined with thymosin a1 treatment is more effective in improving the nutritional status of patients,and can obtain higher overall clinical efficacy,which is worth promoting.
