摘要
目的探讨双歧杆菌活菌胶囊对老年胃食管反流病(GERD)患者长期应用质子泵抑制剂(PPIs)的疗效及肠道菌群的影响。方法选择2016年3月—2018年3月涟水县人民医院接诊的长期服用PPIs的老年GERD患者80例,通过随机数表法分为观察组(=40)和对照组(=40),并选择同期无GERD且未服用PPIs的健康体检者为健康对照组(=50)。对照组继续采取原有的PPIs治疗,观察组在对照组治疗基础上联合双歧杆菌活菌胶囊进行治疗。评估并比较3组纳入研究时肠道菌群数量、观察组和对照组治疗后肠道菌群数量的变化及相关肠道并发症发生率。结果治疗前,观察组和对照组双歧杆菌和乳酸杆菌数量均明显低于健康对照组,而葡萄球菌和肠埃希菌数量均明显高于健康对照组,差异均具有统计学意义(P<0.05);3组大肠杆菌、肠球菌和产气荚膜梭菌数量差异均无统计学意义(P>0.05)。治疗后,观察组双歧杆菌和乳酸杆菌数量明显增加,葡萄球菌和肠埃希菌数量明显降低(P<0.05),且双歧杆菌和乳酸杆菌数量均明显高于对照组(P<0.05),葡萄球菌和肠埃希菌数量均明显低于对照组(P<0.05);观察组腹泻、腹痛腹胀、炎症性肠病、高胃泌素血症等并发症总发生率均明显低于对照组(P<0.05)。结论长期服用PPIs的老年GERD患者采取双歧杆菌活菌胶囊联合治疗,疗效显著;双歧杆菌活菌胶囊有助于调节肠道菌群紊乱,减少相关并发症的发生风险,具有一定的临床应用价值。
Objective To investigate the effect of bifidobacterium capsules on the elderly with gastroesophageal reflux disease reflux disease(DERD)undergoing long-term proton pump inhibitors(PPI)treatment and the impact on intestinal flora.Methods 80 GERD elder undergoing long-term PPI treatment during the period from March,2016 to March,2018 were enrolled and randomly divided into 2 groups:observation group and the control group,40 in each;50 norm of physical examination with no GERD and PPI treatment were selected in blank control group;PPI treatment was kept on in control group and observation group while bifidobacterium capsules were added to the elderly in observation group;comparative evaluation was made to the intestinal flora numbers of the cases in the 3 groups when enrolled,the changes of intestinal flora numbers in the elderly in observation group and control group after treatment and the occurrence of related intestinal complications.Results The numbers of Bifidobacterium and Lactobacillus were significantly lower and the numbers of Staphylococcus and E.coli significantly higher in the elderly in observation group and control group than those in blank control group(P<0.05);there was no statistical differences in the numbers of bacillus coli,Enterococcus and Clostridium perfringens among the 3 groups(P>0.05);after treatment,the numbers of Bifidobacterium and Lactobacillus significantly increased and the numbers of Staphylococcus and E.coli significantly decreased in observation group(P<0.05)while the numbers of Bifidobacterium and Lactobacillus were significantly higher and the numbers of Staphylococcus and E.coli significantly lower in observation group than those in control group(P<0.05);the occurrences of diarrhea,abdominal pain and distention,inflammatory bowel disease and hypergastrinemia in observation group were significantly lower than those in control group(P<0.05).Conclusions Bifidobacterium Capsules is of good curative effect on GERD elderly undergoing long-term PPI treatment by regulating intestinal f
