摘要
目的探讨经尿道前列腺电切术(TURP)对大体积前列腺增生患者膀胱功能、勃起功能及术后并发症的影响。方法我院收治的50例大体积前列腺增生患者,均接受TURP治疗,分析患者手术治疗相关情况、治疗前后膀胱功能[残余尿量(PVR)、最大尿流率(Qmax)、膀胱顺应性(BC)]、勃起功能[国际勃起功能问卷-5(IEF-5)评估]及并发症发生情况。结果本组50例患者,手术时间(95.23±16.58)min,术中出血量(75.06±7.26)ml,住院时间(6.89±2.01)d;术后PVR较术前明显降低,Qmax、BC较术前明显升高(P<0.05);术后3、6月IEF-5评分较术前明显降低(P<0.05)。术后6例出现并发症,发生率12.00%。结论TURP对大体积前列腺增生患者的膀胱功能、勃起功能有一定影响,可明显优化患者手术相关指标,在熟练掌握手术操作技术的前提下,有望成为大体积前列腺增生患者的一种安全有效的治疗术式。
Objective To investigate the effects of transurethral resection of prostate( TURP) on bladder function,erectile function and postoperative complications in patients with large volume prostatic hyperplasia.Methods A total of 50 patients with large volume prostatic hyperplasia admitted to our hospital were enrolled.All patients underwent TURP.The surgical treatment related conditions,bladder functions such as postvoid residual urine( PVR),maximum flow rate( Qmax) and bladder compliance( BC),erectile function such as international index of erectile function-5( IEF-5) and occurrence of complications before and after treatment were analyzed.Results Of the 50 patients,average operation time,average intraoperative blood loss and average hospitalization time were( 95. 23±16. 58) min,( 75. 06±7. 26) ml and( 6. 89±2. 01) d,respectively.After surgery,PVR was significantly decreased,while Qmaxand BC were significantly increased( P < 0. 05).After 3 and 6 months of surgery,IEF-5 score in the patients was significantly decreased( P< 0. 05).Of the 50 patients,there were 6 cases with complications after surgery,and the incidence of complications was 12. 00%.Conclusion There are certain effects of TURP on bladder function and erectile function in patients with large volume prostatic hyperplasia.However,it can significantly optimize the surgical related indexes and reduce the occurrence of complications.Under the premise of expertly mastering surgical operation technique,it is expected to become a safe and effective treatment method for patients with large volume prostatic hyperplasia.
